Ninety percent of city populations died; 2) "The Dancing Mania" tells of a social phenomenon involving groups of people dancing erratically, sometimes thousands at a time. Affecting thousands of people across several centuries, dancing mania was not an isolated event. However, its causes were never explained; 3) "The Sweating Sickness" was a mysterious and contagious disease that struck England and later continental Europe in a series of epidemics beginning in 1485. Yellow fever was a proximate cause of the American invasion of Cuba in 1898, and its control enabled a continued U.S. presence on the island and in the rest of the Caribbean. Had it not been for influenza, German Gen. Erich Ludendorff might well have succeeded in his offensive in the closing years of World War I. The purpose of this paper is to fully define the role of quarantine as a battle-tested public health strategy and its potential impact on military operations in the United States Air Force (USAF). The primary emphasis will focus on how a commander can carry out effective operations while adhering to quarantine requirements during the initial 72-hour period following the realization of an outbreak. Quarantine may not be appropriate in all cases, however this may not be clear until a definitive diagnosis is known, which takes time. Unfortunately, time is not on the side of a commander forced with making a decision on how best to respond to an emerging disease outbreak. From a medical perspective, the initial 72-hour period was selected as a reasonable, conservative window for intervention based on recent bioterrorism exercises. This period of assessment will most likely precede arrival of additional support and formal identification of the involved agent, especially at an overseas installation. Actions taken during this critical period will determine if a commander has any hope of containing spread after a biologic attack. Disease containment challenges faced by a commander at an overseas installation are greater than in the Continental United States (CONUS) based on limited resources with a higher level of threat, so recommendations will focus on quarantine implementation in the overseas environment. <p>This study of disease among the native peoples of the New World before and after 1492 challenges many widely held notions about encounters between European and native peoples. Whereas many late twentieth century scholars blamed the catastrophic decline of postconquest native populations on the introduction of previously unknown infections from the Old World, Alchon argues that the experiences of native peoples in the New World closely resembled those of other human populations. Exposure to lethal new infections resulted in rates of morbidity and mortality among native Americans comparable to those found among Old World populations.</p> <p>Why then did native American populations decline by 75 to 90 percent in the century following contact with Europeans? Why did these populations fail to recover, in contrast to those of Africa, Asia, and Europe? Alchon points to the practices of European colonialism. Warfare and slavery increased mortality, and forced migrations undermined social, political, and economic institutions.</p> <p>This timely study effectively overturns the notion of New World exceptionalism. <p><strong>A History of Epidemics in Britain (Volume I of II) : from A.D. 664 to the Extinction of Plague</strong></p> <p>The Middle Age of European history has no naturally fixed beginning or ending. The period of Antiquity may be taken as concluded by the fourth Christian century, or by the fifth or by the sixth; the Modern period may be made to commence in the fourteenth, or in the fifteenth or in the sixteenth. The historian Hallam includes a thousand years in the medieval period, from the invasion of France by Clovis to the invasion of Italy by Charles VIII. in 1494. We begin, he says, in darkness and calamity, and we break off as the morning breathes upon us and the twilight reddens into the lustre of day. To the epidemiologist the medieval period is rounded more definitely. At the one end comes the great plague in the reign of Justinian, and at the other end the Black Death. Those are the two greatest pestilences in recorded history; each has no parallel except in the other. They were in the march of events, and should not be fixed upon as doing more than their share in shaping the course of history. But no single thing stands out more clearly as the stroke of fate in bringing the ancient civilization to an end than the vast depopulation and solitude made by the plague which came with the corn-ships from Egypt to Byzantium in the year 543; and nothing marks so definitely the emergence of Europe from the middle period of stagnation as the other depopulation and social upheaval made by the plague which came in the overland track of Genoese and Venetian traders from China in the year 1347. While many other influences were in the air to determine the oncoming and the offgoing of the middle darkness, those two world-wide pestilences were singular in their respective effects: of the one, we may say that it turned the key of the medieval prison-house; and of the other, that it unlocked the door after eight hundred years.</p> <p> </p> <p>The Black Death and its after-effects will occupy a large part of this work, so that what has just been said of it will not stand as a bare assertion. But the plague in the reign of Justinian hardly touches British history, and must be left with a brief reference. Gibbon was not insensible of the part that it played in the great drama of his history. “There was,” he says, “a visible decrease of the human species, which has never been repaired in some of the fairest countries of the globe.” After vainly trying to construe the arithmetic of Procopius, who was a witness of the calamity at Byzantium, he agrees to strike off one or more ciphers, and adopts as an estimate “not wholly inadmissible,” a mortality of one hundred millions. The effects of that depopulation, in part due to war, are not followed in the history. So far as Gibbon’s method could go, the plague came for him into the same group of phenomena as comets and earthquakes; it was part of the stage scenery amidst which the drama of emperors, pontiffs, generals, eunuchs, Theodoras, and adventurers proceeded. Even of the comets and earthquakes, he remarks that they were subject to physical laws; and it was from no want of scientific spirit that he omitted to show how a plague of such magnitude had a place in the physical order, and not less in the moral order.</p> <p> </p> <p>A new science of epidemiology has sprung up since the time of Gibbon, who had to depend on the writings of Mead, a busy and not very profound Court physician. More particularly the Egyptian origin of the plague of the sixth century, and its significance, have been elucidated by the brilliant theory of Pariset, of which some account will be given at the end of the chapter on the Black Death. For the present, we are concerned with it only in so far as it may have a bearing upon the pestilences of Britain. The plague of the sixth century made the greatest impression, naturally, upon the oldest civilized countries of Europe; but it extended also to the outlying provinces of the empire, and to the countries of the barbarians. This report explores the emerging epidemics of HIV among men who have sex with men (MSM) in low and middle income countries (LMIC). A case study approach is used to explore these epidemics in detail in Peru Ukraine Kenya and Thailand. Systematic reviews of epidemiology and prevention science were used to understand these epidemics and to assess the evidence for interventions in prevention access to care and treatment and human rights protections. The Goals model was then used to assess the potential impact at country level of responding to epidemics among MSM. Benefits were seen for overall HIV epidemic control in all four countries with greater investment in MSM prevention treatment and care. Costing and cost-effectiveness work found these interventions to be cost effective. The population of the world grew from 1 billion in 1800 to 7 billion in 2012, and over the past 50 years the mean temperature has risen faster than ever before. Both factors continue to rise, as well as health inequalities. Our environment is changing rapidly, with tremendous consequences for our health. These changes produce complex and constantly varying interactions between the biosphere, economy, climate and human health, forcing us to approach future global health trends from a new perspective. Preventive actions to improve health, especially in low-income countries, are essential if our future is going to be a sustainable one. After a period of undeniable improvement in the health of the world’s population, this improvement is likely to slow down and we will experience? at least locally ? crises of the same magnitude as have been observed in financial markets since 2009. There is instability in health systems, which will worsen if preventive and buffering mechanisms do not take on a central role. We cannot exclude the possibility that the allied forces of poverty, social inequalities, climate change, industrial food and lack of governance will lead to a deterioration in the health of large sectors of the population. In low-income countries, while many of the traditional causes of death (infectious diseases) are still highly prevalent, other threats typical of affluent societies (obesity, diabetes, cardiovascular diseases) are increasing. Africa is not only affected by malaria, TB and HIV, but also by skyrocketing rates of cancer. The book argues that the current situation requires effective and coordinated multinational interventions guided by the principle of <em>health as a common good</em>. An entirely competition-driven economy cannot ? by its very nature ? address global challenges that require full international cooperation.
