Die NIDEP-Studie (Nosokomiale Infektionen in Deutschland - Erfassung und Pr?vention) untersuchte die nosokomialen Infektionen und fand heraus, dass etwa 3,5% aller Patienten im Krankenhaus sich nosokomial infizierten, auf Intensivstationen lag die Rate sogar bei 15%. Laut des Krankenhaus-Infektions-Surveillance-Systems (KISS) von 2003 sind Pneumonien die h?ufigste nosokomiale Infektion in Deutschland mit einer Rate von 48%. Das Risiko an einer Pneumonie zu erkranken steigt erheblich an, wenn der Patient maschinell beatmet wird. Aufgrund der Intubation wird eine direkte Verbindung zum respiratorischen Trakt hergestellt, dies beg?nstigt das Eindringen pathogener Keime aus dem Mundrachenraum. Eine beatmungsassoziierte Pneumonie f?hrt, je nach Studie, zu einem verl?ngerten Aufenthalt auf der Intensivstation von 5-13 Tagen und steigert die Mortalit?t um 16-39%. Insgesamt bel?uft sich die j?hrliche Infektionsrate der beatmungsassozierten Pneumonie auf 22.000. Von diesen 22.000 Patienten sterben ca. 10%, das hei?t pro Jahr versterben etwa 2.200 Patienten an den Folgen einer beatmungsassoziierten Pneumonie. Diese Fakten verdeutlichen, dass es unabdingbar ist vermeidbare Infektionen zu erkennen und ihr Auftreten zu verhindern. Einer Studie aus 2007 zufolge werden mangelnde orale Hygiene und oropharyngealer Kolonisation mit pathogenen Keimen direkt in Verbindung mit dem Auftreten von Pneumonien gebracht. Diese Ergebnisse zeigen auf, dass Mundpflege eine wichtige Prophylaxema?nahme f?r eine nosokomiale Pneumonie ist. In der Pflege werden jedoch verschiedenste Mundpflegeartikel benutzt und Mundpflegema?nahmen durchgef?hrt, in manchen Kliniken gibt es Leitlinien, nach denen die Mitarbeiter verfahren sollen, in anderen f?hrt sie jeder einzelne nach bestem Gewissen durch. Im Rahmen einer evidenzbasierten Pflege m?ssen wissenschaftlich fundierte Aussagen zur Verf?gung stehen, die Pflegekr?ften die M?glichkeit gibt, aktiv nosokomiale Pneumonien zu verhindern.
