It shows by statistical studies and illustrative case histories that the basic cause is the survival into old age of people who are unfit to care for themselves, in such numbers that help from families, neighbours, the social services and the NHS is insufficient. From this study the expression the "geriatric giants" or the four I’s was coined: impairment of intellect (cerebral dysfunction), incontinence, immobility and instability (falls). The term ‘giant’ is seen to refer both to statistical frequency and to the huge personal burden of sufferers, escalating the need for socio-medical intervention. Standing where the three roads meet at the old turnpike-gate of the county, the square brick building, which had declined through several generations from a chapel into a tavern, had grown at last to resemble the smeared face of a clown under a steeple hat which was worn slightly awry. Originally covered with stucco, the walls had peeled year by year until the dull red of the bricks showed like blotches of paint under a thick coating of powder. Over the wide door two little oblong windows, holding four damaged panes, blinked rakishly from a mat of ivy, which spread from the rotting eaves to the shingled roof, where the slim wooden spire bent under the weight of creeper and innumerable nesting sparrows in spring. After pointing heavenward for half a century, the steeple appeared to have swerved suddenly from its purpose, and to invite now the attention of the wayfarer to the bar beneath. This cheerful room which sprouted, like some grotesque wing, from the right side of the chapel, marked not only a utilitarian triumph in architecture, but served, on market days to attract a larger congregation of the righteous than had ever stood up to sing the doxology in the adjoining place of worship. Good and bad prospects were weighed here, weddings discussed, births and deaths recorded in ever-green memories, and here, also, were reputations demolished and the owners of them hustled with scant ceremony away to perdition. From the open door of the bar on this particular October day, there streamed the ruddy blaze of a fire newly kindled from knots of resinous pine. Against this pleasant background might be discerned now and then the shapeless silhouette of Betsey Bottom, the innkeeper, a soft and capable soul, who, in attaching William Ming some ten years before, had successfully extinguished his identity without materially impairing her own. Bottom's Ordinary had always been ruled by a woman, and it would continue to be so, please God, however loudly a mere Ming might protest to the contrary. In the eyes of her neighbours, a female, right or wrong, was always a female, and this obvious fact, beyond and above any natural two-sided jars of wedlock, sufficed in itself to establish Mrs. Ming as a conjugal martyr. Being an amiable bodyーpeaceably disposed to every living creature, with the exception of Williamーshe had hastened to the door to reprimand him for some trivial neglect of the grey mule, when her glance lighted upon the stranger, who had come a few minutes earlier by the Applegate road. A casual observer might have classified him as one of the Virginian landowners impoverished by the war; in reality, he was a successful lawyer in a neighbouring town, who, amid the overthrow of the slaveholding gentry some twenty years before, had risen into a provincial prominence. His humour met with a slow response from the driver, who sat playfully flicking at a horsefly on the flank of a tall, raw-boned sorrel. "Wall, thar's been a sight of rain lately," he observed, with goodnatured acquiescence, "but I don't reckon the mud's more'n waist deep, an' if you do happen to git clean down, thar's Sol Peterkin along to pull you out. Whar're you hidin', Sol? Why, bless my boots, if he ain't gone fast asleep!" At this a lean and high-featured matron, encased in the rigidity of her Sunday bombazine, gave a prim poke with her umbrella in the ribs of a sparrow-like little man, with a discoloured, scraggy beard, who nodded in one corner of the long seat. "I'd wake up if I was you," she remarked in the voice her sex assumes when virtue lapses into severity. Starting from his doze, the little man straightened his wiry, sunburned neck and mechanically raised his hand to wipe away a thin stream of tobacco juice which trickled from his half-open mouth. "Hi!we ain't got here a'ready!" he exclaimed, as he spat energetically into the mud. "I d'clar if it don't beat allーone minute we're thar an' the next we're here. It's a movin' world we live in, ain't that so, mum?" Her face was very careworn and very sad, her eyes red and dim with long weeping. There was not on the face of the earth a more desolate creature than the gentle, slender girl, the orphan of a day. At an age when life should be a joyous and lovely thing to the maiden child, Gladys Graham found herself face to face with its grimmest reality, certain of only one thing, that somewhere and somehow she must earn her bread. She was thinking of it at that moment, with her white brows perplexedly knitted, her mouth made stern by doubt and apprehension and despair; conning in her mind her few meagre accomplishments, asking herself how much they were likely to bring in the world's great mart.
