<p>In the 1960s, psychiatry was entering a new phase of its development. However, many people were uncomfortable with these changes and fought against what they saw as apostasy, dangerous, and unprofessional decisions by the bureaucracy. The role of Nurses before 1960 was task-oriented, to control behaviour, give somatic treatments when ordered, and provide the daily personal needs of their patients, a custodial model. Since 1960, the nursing profession, ancillary staff and Doctors have been forced to reassess their traditional involvement and develop new skills. All previous somatic treatments were abandoned or rarely used in mainstream psychiatry. The only tools left for nurses were consciously using self in a therapeutic relationship, communicating, sharing, developing empathy and compassion, and helping the patient gain insight, self-awareness and problem-solving skills. No longer were the staff seen as the ‘boss’, demanding obedience. Nurses had to learn how to work in alternative environments, halfway houses, day clinics, community clinics, night hospitals, acute units, work with mixed sexes, and learn new skills through family therapy, TA, cognitive behaviour modification, encounter groups, gestalt therapy, reality therapy and a therapeutic community and adapt to different age and cultural groups. Management of wards and staff presented a challenge to administrators. In-patients attained much more freedom and self-government with these developments. The hierarchy was flatter, industrial democracy was expected, and workers had to cope with changing organisational structures. Government decision-making was a constant source of conflict. A new approach to management emerged, trust replaced control, and self-government replaced regimentation. Where were all these developments leading? What of the future? The story of the Yarmouth Castle disaster is a part of the fabric of the story of PMH, a story that begins in the 1780s with the opening of the African Hospital in Nassau. We trace the evolution of the Poor House on Shirley Street into the Bahamas General Hospital, forerunner of the PMH. Along the way we describe a commission of inquiry into hospital corruption (1915), a disruptive doctors strike and even the murder of a hospital nurse on the private ward. We knew that our investigations were probing sensitive areas when officials had difficulty locating reports and photographs. Nevertheless, research trumped resistance. We interviewed pioneers and disaster survivors, studied documents in the National Archives of the Bahamas, the Supreme Court registry, the Ministry of Health and even the library of the United States Coast Guard. Princess Margaret Hospital is much more than a sleepy account of the construction of an old hospital. Built to serve St. Louis’s rapidly expanding African-American population, the grand new hospital opened its doors in 1937, toward the end of the Great Depression. “Homer G.,” as many called it, joined a burgeoning group of black hospitals amid a national period of institutional segregation and strong racial prejudice nationwide.</p> <p>When the beautiful, up-to-date hospital opened, it attracted more black residents than any other such program in the United States. Patients also flocked to the hospital, as did nursing students who found there excellent training, ready employment, and a boost into the middle class. For decades, the hospital thrived; by the 1950s, three-quarters of African-American babies in St. Louis were born at Homer G.</p> <p>But the 1960s and 1970s brought less need for all-black hospitals, as faculty, residents, and patients were increasingly welcome in the many newly integrated institutions. Ever-tightening city budgets meant less money for the hospital, and in 1979, despite protests from the African-American community, HGPH closed. <p>In response to the 1845 Lunacy Act, initial, and what appeared to be perfunctory discussions took place in 1846 on the need for Newcastle to build its own asylum for pauper lunatics. It wasn’t until 1863 however, that proper consideration was given for the first time on whether the city should indeed build its own asylum or at least look into possible alternatives. When it eventually opened in 1869, the high ideals associated with such a venture were superseded almost from the outset by the need for enlargement to address the continual problems of overcrowding. This subsequently led to an almost constant programme of expansion that saw the asylum grow ever bigger in size over the next few decades.</p> <p>In the 1960’s ? almost one hundred years later ? proposals were put forward for a programme of closure that would herald the end of the asylum era. These proposals, in effect, were to be the precursor of care in the community initiatives which would eventually see the demise of mental hospitals such as St. Nicholas - although this would take many years to come to fruition. The physical manifestation of this process, for example, only began to have an impact from the early 1980’s onwards through the gradual contraction and displacement of hospital services as they became increasingly community-based.</p> <p>St Nicholas Hospital has had a long and varied history in its role as both lunatic asylum and psychiatric hospital. Nevertheless, despite various references to its presence in a number of local histories, its past has never been fully investigated in any great depth ? until now.
