|
Pediatricians care for children and families from all walks of life. Some are children known from neighborhoods. Others are children from distant lands. Pediatric focus does not stop with the physical care of children but extends to include their mental and social-emotional health and concern about their families. Pediatricians care about how children are doing at home, at school, and in their communities. In this era, children and their families are impacted by social and political changes in their homes (social media and screen time), in their communities (refugee populations and children requiring palliative supports at school), in their health care networks (EMR in every tertiary pediatric center), and in the larger world (multiple military deployments of fathers and mothers). This issue explores the impact of contemporary public health challenges for pediatric care, promising models for caring for chronically ill children, and state of the art therapies for complex childhood conditions.

|
Pediatricians in the Unites States and around the World continue to face a myriad of global health threats affecting child and adolescent health including: 1) infectious diseases of poverty [e.g. human immunodeficiency virus (HIV), tuberculosis, malaria, and neglected tropical diseases] in low-and middle-income countries (LMIC), especially in sub Saharan Africa; 2) emerging and reemerging infectious diseases (such as Ebola); 3) rise of non-communicable diseases (e.g. common mental disorders); 4) unintentional injuries; and 5) environmental health hazards (e.g. climate change). Despite the promising news about rapid declines in maternal and child mortality in the era of Millennium Development Goals (MDGs), which originated from the United Nation (UN) Millennium Declaration in 2000, only 20% and 7% of LMIC are currently on track to attain the maternal and child mortality targets. For example, 44% of deaths in children younger than 5 years occur in neonates. Besides discovery of life-saving interventions (e.g. development of new and improved vaccines) for maternal and child health, we also need to do a better job at bridging the knowledge-implementation gap and increase the effectiveness of proven interventions. For example, despite the availability of effective vaccines to prevent pneumococcal pneumonia, rotavirus gastroenteritis, and human papilloma virus-related diseases (e.g. cervical cancer), use of these vaccines remain suboptimal in LMIC. We need to recognize that global health is also local public health. For example, improving access, equity and quality of care for orphans and vulnerable children, immigrant and newly arrived refugee children in the U.S. remains a challenge. Timely access to psychiatric care for children and adolescents with mental illness is a major concern. The explosion of new age technology (such as the internet) also poses a considerable risk to children and adolescents. Pediatricians also need to be aware of diverse socio-cultural determinants of health and ethical issues in global health service and delivery. This issue of Pediatric Clinics of North America aims to address the above crucial global health challenges affecting children and adolescents. As practicing pediatricians, we have the unique opportunity to influence local and global public health. In the post-MDG era (beyond 2015), collaborative partnerships between various disciplines and across research, education and service is vital to reduce health inequities in children worldwide.

| Pediatric Speech and Language: Perspectives on Interprofessional Practice, An Issue of Pediatric Clinics of North America【電子書籍】[ Brian B. Shulman, PhD, CCC-SLP, ASHA Fellow, BCS-CL, ASAHP ]
|
Like many other medical and health-related professions, the speech-language pathology and audiology professions have been responding to the changes in the ways in which healthcare is delivered. These changes are occurring at the federal and state levels due to changes in legislative and regulatory policies. To this end and as a result of reports from the World Health Organization (2010, 2011, 2013) and the Institute of Medicine (2000, 2001, 2015), healthcare professionals are now addressing healthcare service delivery within an interprofessional, collaborative practice and patient outcomes context. This volume of Pediatric Clinics will present articles that address clinical care to a variety of pediatric speech and language clinical populations. While it will be the intent of each piece to address interprofessional perspectives of care, the opening and closing pieces, respectively, will introduce and then synthesize, all of the articles into a cogent volume. Articles are devoted to the following topics: The Journey to Interprofessional Practice: Are We There Yet; Late Talkers: Why the ‘Wait and See’ Approach is Outdated; A Pediatrician’s Guide to Cleft Palate Speech and Non-Cleft Causes of Velopharyngeal Insufficiency (VPi); Supporting Children with Autism and Their Families: A Model for Interprofessional Practice; Feeding Problems in Infants and Children: Assessment and Etiology; A Multidisciplinary Team Approach to the Differential Diagnosis of Children with Communication Disorders; Open Up and Let Us In: An Interdisciplinary Approach to Oral Health; Otitis Media: Beyond the Examining Room; Clinical Perspectives on Pre-Literacy Development in Young Children; Using the ICF Framework to Achieve Interprofessional Functional Outcomes for Young Children: A Speech-Language Pathology Perspective; Developmental Care of the Pre-Term Infant: From NICU Through High-Risk Infant Follow Up; Interprofessional Collaborative Practice: What That Means for Early Intervention Service Delivery; The Role of the Speech-Language Pathologist in Creating a Model for Interprofessional Practice in an Ambulatory Care Clinic; and Identification of Speech and Language Delays in Young Children: An Important Collaborative Role for the Pediatrician.

| Pediatric Hospital Medicine and Pediatric Palliative Care, An Issue of Pediatric Clinics【電子書籍】[ Mary C. Ottolini, MD ]
|
For the first time, Pediatric Clinics is devoting one issue to two clinically focused topics: Pediatric Palliative Care and Pediatric Hospital Medicine. Dr. Ottolini has organized her section to focus on a variety of issues of relevant to all pediatricians, but which pose special challenge to the Pediatric Hospitalists. As pediatric care has advanced, children who would not have survived infancy are growing into young adults with complex chronic diseases and dependence upon technology. They frequently require hospitalization to address exacerbation of underlying disease processes and procedures to improve their quality of life. The articles are devoted to patient care challenges of troubleshooting malfunctioning technology, co-managing medically complex patients pre and post-op with surgical colleagues, and the Hospitalist’s evolving role in performing procedures and sedation in this population of vulnerable patients. Also discussed rare strategies to maximize communication with parents, patients and primary care providers during hospitalization, especially for medically complex patients. Dr. Ullrich and Dr. Wolfe Pediatric have worked to bring relevant articles on palliative care to the pediatrician. While life-threatening conditions in childhood are rare, children with LTC account for a high proportion of pediatric hospital care, and about one half of such children die in the inpatient setting. The number of hospital-based pediatric palliative care programs has increased dramatically over the past decade to meet the palliative care needs of hospitalized children including symptom management, facilitation of communication, decision-making and advance care planning support, and coordination of care. Given these considerations, it is evident that the topics of pediatric palliative care and hospital medicine are fitting counterparts for this comprehensive issue.
