<p>The 1979 nuclear meltdown accident at the Three Mile Island (TMI) plant in Pennsylvania is fully covered in this authoritative collection of official documents with details about the accident and its aftermath, including the immediate and long-term health effects, a full reproduction of the report of the President's Commission on the Accident at TMI, detailed timelines of the accident with technical information on the accident, fuel core meltdown, the evacuations, political reactions, media reports, and public consequences, and much more.</p> <p>The accident began about 4:00 a.m. on March 28, 1979, when the plant experienced a failure in the secondary, non-nuclear section of the plant. The main feedwater pumps stopped running, caused by either a mechanical or electrical failure, which prevented the steam generators from removing heat. First the turbine, then the reactor automatically shut down. Immediately, the pressure in the primary system (the nuclear portion of the plant) began to increase. In order to prevent that pressure from becoming excessive, the pilot-operated relief valve (a valve located at the top of the pressurizer) opened. The valve should have closed when the pressure decreased by a certain amount, but it did not. Signals available to the operator failed to show that the valve was still open. As a result, cooling water poured out of the stuck-open valve and caused the core of the reactor to overheat. As coolant flowed from the core through the pressurizer, the instruments available to reactor operators provided confusing information. There was no instrument that showed the level of coolant in the core. Instead, the operators judged the level of water in the core by the level in the pressurizer, and since it was high, they assumed that the core was properly covered with coolant. In addition, there was no clear signal that the pilot-operated relief valve was open. As a result, as alarms rang and warning lights flashed, the operators did not realize that the plant was experiencing a loss-of-coolant accident. They took a series of actions that made conditions worse by simply reducing the flow of coolant through the core. Because adequate cooling was not available, the nuclear fuel overheated to the point at which the zirconium cladding (the long metal tubes which hold the nuclear fuel pellets) ruptured and the fuel pellets began to melt. It was later found that about one-half of the core melted during the early stages of the accident. Although the TMI-2 plant suffered a severe core meltdown, the most dangerous kind of nuclear power accident, it did not produce the worst-case consequences that reactor experts had long feared. In a worst-case accident, the melting of nuclear fuel would lead to a breach of the walls of the containment building and release massive quantities of radiation to the environment. <p>Industrial activities like textile processing and mining are typical sources of heavy metal-rich wastewaters. The sulfate reducing process has become an attractive method for the production of sulfide to precipitate metals since most of these streams also contain sulfate, which is the electron acceptor and, in less common cases, chemical oxygen demand which is the electron donor of sulfate reducing bacteria. The inverse fluidized bed (IFB) reactor is a system for the production of biogenic sulfide and metal precipitation in the same unit due to its configuration: the biomass floats on top of the reactor, whereas metal sulfide precipitates settle and thus can be recovered at the bottom. The main objective of this thesis was to elucidate the factors affecting simultaneous sulfate reduction and precipitation of heavy metals in an IFB reactor in order to optimize the metal recovery from wastewaters such as acid mine drainage. Therefore, this thesis focused on varying different operational conditions to study their effect on the solid-liquid separation and purity of the metal sulfide precipitates as well as on their effect on the sulfate reducing process. Furthermore, one chapter was focused on the study of strategies for sulfide control in the IFB reactor. <p>The Near-Term Task Force was established in response to Commission direction to conduct a systematic and methodical review of U.S. Nuclear Regulatory Commission processes and regulations to determine whether the agency should make additional improvements to its regulatory system and to make recommendations to the Commission for its policy direction, in light of the accident at the Fukushima Dai-ichi Nuclear Power Plant. The Task Force appreciates that an accident involving core damage and uncontrolled release of radioactive material to the environment, even one without significant health consequences, is inherently unacceptable. The Task Force also recognizes that there likely will be more than 100 nuclear power plants operating throughout the United States for decades to come. The Task Force developed its recommendations in full recognition of this environment. In examining the Fukushima Dai-ichi accident for insights for reactors in the United States, the Task Force addressed protecting against accidents resulting from natural phenomena, mitigating the consequences of such accidents, and ensuring emergency preparedness. The accident in Japan was caused by a natural event (i.e., tsunami) which was far more severe than the design basis for the Fukushima Dai-ichi Nuclear Power Plant. As part of its undertaking, the Task Force studied the manner in which the NRC has historically required protection from natural phenomena and how the NRC has addressed events that exceed the current design basis for plants in the United States.</p> <p>The Task Force finds that the Commission's longstanding defense-in-depth philosophy, supported and modified as necessary by state-of-the-art probabilistic risk assessment techniques, should continue to serve as the primary organizing principle of its regulatory framework. The Task Force concludes that the application of the defense-in-depth philosophy can be strengthened by including explicit requirements for beyond-design-basis events. Many of the elements of such a regulatory framework already exist in the form of rules regarding station blackout, anticipated transient without scram, maintenance, combustible gas control, aircraft impact assessment, beyond-design-basis fires and explosions, and alternative treatment. Other elements, such as severe accident management guidelines, exist in voluntary industry initiatives.
