Cochlear Implants and Hearing Preservation【電子書籍】[ Patrick J. Bradley ]
|

Electric acoustic stimulation (EAS) combines electric stimulation in the mid- to high-frequency regions with acoustic stimulation in the low-frequency range with the aim to preserve residual low-frequency hearing after cochlear implantation, which together particularly improves speech understanding, pitch discrimination and music appreciation. In this volume, the most experienced clinical groups share their understanding of the use of EAS in adults and children. It offers an in-depth audiological analysis related to selecting, preparing and rehabilitating EAS patients. Topics such as dead zone assessment, psychophysics of low-frequency hearing, electric-acoustic interaction, speech algorithms, music perception, as well as fitting and the patient’s acceptance are discussed. Introductory chapters - illustrated with exceptional colour images - on cochlear neural reserves, molecular biology and high-technological electrode development focus on the basic scientific EAS research. Every ENT specialist, audiologist, speech therapist and scientist interested in inner ear pathology, involved in cochlear implantation or dealing with the treatment or surgery of the inner ear will benefit from the insights and experiences of the world’s leading experts who contributed to this publication.

Sensory Futures Deafness and Cochlear Implant Infrastructures in India【電子書籍】[ Michele Ilana Friedner ]
|

Revealing inequalities and sensory hierarchies embedded in the latest medical technologies and global biotechnical markets

What happens when cochlear implants, heralded as the first successful bionic technologies, make their way around the globe and are provided by both states and growing private markets? As Sensory Futures follows these implants from development to domestication and their unequal distribution in India, Michele Ilana Friedner explores biotechnical intervention in the realm of disability and its implications for state politics in the Global South.

A signing and speaking deaf bilateral cochlear implant user, Friedner weaves personal reflections into this fine-grained ethnography of everyday negotiations, activist aspirations, and the space of the family. She places sensory anthropology in conversation with disability studies to analyze how normative sensoria are cultivated and the pursuit of listening and speaking capability is enacted. She argues that the conditions of potentiality that have emerged through cochlear implantation have, in fact, resulted in ever narrower understandings of future life possibilities. Rejecting sensory hierarchies that privilege audition, Friedner calls for multisensory, multimodal, and multipersonal ways of relating to the world.

Sensory Futures explores deaf people’s desires to create habitable worlds and grapple with what their futures might look like, in India and beyond, amid a surge in both biotechnical interventions and disability rights activism. With implications for a broad range of disability experiences, this sensitive, in-depth research focuses on the specific experiences of deaf people, both children and adults, and the structural, political, and social possibilities offered by both biotechnological and social “cures.”

Cochlear Implants Evolving Perspectives【
|

The cochlear implant debate has changed, as evidenced in this cogent collection that presents 13 chapters by 20 experts, including several who communicate through sign language but also utilize cochlear implants. The impetus for this change stems from recognition that both visual and aural input can enhance the education of deaf children.

Divided into four sections, Cochlear Implants: Evolving Perspectives first focuses on the impact of implants in the Deaf community. Chapters in this section examine the issues driving the cochlear implant debate, the ethics of genetic engineering, experiences of implanted adult deaf signers, reflections of deaf mothers who have had their children implanted, and the effects of implants on deaf identity. The second section delves into the mechanics of bimodal processing, including listening strategies that can benefit signing children with cochlear implants. The third section surveys combined aural/visual educational approaches, such as teaching implanted children in an ASL/English bilingual classroom, and applying auditory rehabilitation to a signed communication context.

The final section challenges readers to reframe the debate first by exploring sensory politics, then by envisioning an emerging world that requires the Deaf community to connect with it to secure its future. With this information, readers will reach their own conclusions about cochlear implants and auditory and visual approaches to the mastery of both spoken and signed languages.

Implantable Hearing Devices other than Cochlear Implants【

Hearing loss can vary in type ranging from conductive, mixed to sensorineural, as well as in degree from mild, moderate, severe to profound. There could also be multiple permutations and combinations like moderate mixed hearing loss or severe conductive hearing loss. In addition, the hearing loss could be unilateral or bilateral. While cochlear implants were devised for bilateral profound sensorineural hearing loss, various other devices have been invented for other types of hearing losses. Research continues to design a suitable implant which would amplify sound for patients who cannot be candidates for cochlear implants.

Made to Hear Cochlear Implants and Raising Deaf Children【電子書籍】[ Laura Mauldin ]
|

A mother whose child has had a cochlear implant tells Laura Mauldin why enrollment in the sign language program at her daughter’s school is plummeting: “The majority of parents want their kids to talk.” Some parents, however, feel very differently, because “curing” deafness with cochlear implants is uncertain, difficult, and freighted with judgment about what is normal, acceptable, and right. Made to Hear sensitively and thoroughly considers the structure and culture of the systems we have built to make deaf children hear.

Based on accounts of and interviews with families who adopt the cochlear implant for their deaf children, this book describes the experiences of mothers as they navigate the health care system, their interactions with the professionals who work with them, and the influence of neuroscience on the process. Though Mauldin explains the politics surrounding the issue, her focus is not on the controversy of whether to have a cochlear implant but on the long-term, multiyear undertaking of implantation. Her study provides a nuanced view of a social context in which science, technology, and medicine are trusted to vanquish disabilityーand in which mothers are expected to use these tools. Made to Hear reveals that implantation has the central goal of controlling the development of the deaf child’s brain by boosting synapses for spoken language and inhibiting those for sign language, placing the politics of neuroscience front and center.

Examining the consequences of cochlear implant technology for professionals and parents of deaf children, Made to Hear shows how certain neuroscientific claims about neuroplasticity, deafness, and language are deployed to encourage compliance with medical technology.

A Polymer Cochlear Electrode Array: Atraumatic Deep Insertion, Tripolar Stimulation, and Long-Term Reliability【電子書籍】[ Tae Mok Gwon ]
|

This book describes the design, fabrication and evaluation of a polymer-based neural interface for a cochlear electrode array, reviewed in terms of fabrication process, functionality, and reliability. Polymer-based devices have attracted attention in the neural prosthetic field due to their flexibility and compatibility with micro-fabrication process. A liquid crystal polymer (LCP) is an inert, highly water-resistant polymer suitable for the encapsulation of electronic components and as a substrate material for fabricating neural interfaces. The author has designed, fabricated, and evaluated an LCP-based cochlear electrode array for an improved polymer-based cochlear implant. The thesis deals with 3 key topics: atraumatic deep insertion, tripolar stimulation, and long-term reliability. Atraumatic insertion of the intracochlear electrode and resulting preservation of residual hearing have become essential in state?of-the-art cochlear implantation. A novel tapered design of anLCP-based cochlear electrode array is presented to meet such goals. For high-density and pitch-recognizable cochlear implant, channel interaction should be avoided. Local tripolar stimulation using multi-layered electrode sites are shown to achieve highly focused electrical stimulation. This thesis addresses another vital issue in the polymer-based neural implants: the long-term reliability issue. After suggesting a new method of forming mechanical interlocking to improve polymer-metal adhesion, the author performs accelerating aging tests to verify the method’s efficacy. The aforementioned three topics have been thoroughly examined through various in vitro and in vivo studies. Verification foresees the development of LCP-based cochlear electrode array for an atraumatic deep insertion, advanced stimulation, and long-term clinical implant.

|

The common cavity deformity (CCD) of the inner ear is characterized by cochlea and vestibule forming a single cavity. It indicates cochlear implantation (CI) and about 15% to 25% of inner ear malformation. However, complications such as facial nerve injury, penetration of the electrode array into the internal auditory canal (IAC) and electrode displacement are common, leading to unsatisfied outcomes. This book describes the embryology of CCD, preoperative auditory, balance and vestibular evaluation, and electrophysiology and radiology aspect for CCD. In addition, the surgical technique and CI outcomes will be focused. The electrode array design is also a highlight for CCD surgery, and will be discussed in details. Last but not least, the balance and vestibular function research of CCD patients before and after surgery is also thoroughly reviewed. It will be a good reference for otologist and practitioners in related field to understand more about CCD.

SoCalDeafMom.com Hearing With Cochlear Implant Cochlear Implant【電子書籍】[ Sandra Caballero ]
|

My name is Sandra Caballero. I will be 39 years old this year in the Covid-19 pandemic time. I'm a mom of two daughters and one son. I love being a mom and yes it's not that easy.

I'm wearing a cochlear implant on my right ear after I lost my hearing completely back in 2000. I woke up one morning and thought my ear mold hearing aid was not working. My audiologist tested the ear mold hearing aid and it was working just fine. I went into the hearing testing booth and the result were that I lost my hearing. For no reason my hearing was gone just like that. I was not sick. At the time I was 20 years old. I was in shock!

My audiologist suggested Cochlear Implant because I was able to understand sounds before I lost my hearing. Also because it was the only solution to hear again. She handed me a Cochlear Implant book. I read everything and did the online review check to see if Cochlear Implant was for me.

It took me weeks before I decided to get the Cochlear Implant. I thought about hearing again and how I would understand the sounds. Since I always spoke and listen to people talked to me. I also thought about the surgery. Would it hurt but pain reliever pills will help. I thought about after surgery that I won't hear anything until my ear is healed and wear the cochlear implant.

I met Dr. De La Cruz at the House Ear Clinic in Los Angeles,California. Great doctor! He discussed with me the process of receiving the Cochlear Implant. ( May He Rest In Peace)

He explained every step of the surgery and after surgery. At the House Ear Clinic I was told that I would be on the waiting list when someone donate money into the House Ear Clinic. One day I got the call that I was accepted to get my Cochlear Implant! I was so excited! Couldn’t wait to hear again! I thank those people who donate to House Ear Clinic.

Surgery day came and was finished by the end of the day. A one day surgery that I got to go home to recover until my right ear heal.

A electrode array was placed inside the cochlear of my right ear and receiver/stimulator inside the side of my head.

When I arrive home from the hospital on the same day of the surgery; I was in so much pain. I could only sleep on one side that was not operated. I felt the pain for few days but the ibuprofen helped so much. Sleeping a lot and eating liquid food also helped. Following the doctor’s orders is the key to the best result.

I recovered completely in a month. I went back to the House Ear Clinic to see Dr.De La Cruz. He checked how the surgical wound healed. He was happy to see that I took care of it very good. Because how nicely and healthy the wound came out. I was able to move to the next step and schedule an appointment with a audiologist at the House Ear Clinic.

I was completely deaf for almost two months. Then I met the audiologist to set up the powerful bodyworn speech processor. The box was placed in my Jean back pocket along with the wires going up to place the implant onto my right ear. This was a huge difference from my old ear mold hearing aid. I was able to hear again! The sound was awful loud and weird. The audiologist adjust from her computer to my processor as it was connected to her computer. I raised my hand if the sound beeps were too loud or too soft. I also had to hear words and say what I heard through my processor. When the audiologist put everything together to get the result of my hearing test. She set up programs. Program 1 would be so loud.. program 2 would be soft and thick.. and program 3 just not clear at all. I would pick the one that comfort my hearing and brain. ( yes; I say brain because listening send signal to the brain to understand what sound you heard. ) The processor is connected to the box where it’s place in the back of my jean pocket. There are buttons to change the programs and volumes. Even batteries to replace when it dies.

So much to adapt when I received the processor. I was stressed but I was strong because I wanted to hear again!

Several appointments may be needed after this initial activation to adjust the program as patients adapt to sound and practice listening with the device. The number of appointments needed varies by the person but is typically 3 to 6 appointments the first year and one appointment per year after that.

I recommend auditory therapist to help reach your full potential with a cochlear implant. It requires a lot of hearing practice and brain connection to understand what sound you are hearing.

I had to ask everyone around me what noise I was hearing. I heard a lot of sounds that I never heard before with my old ear mold hearing aid.

Few months later I receive a speech processor that is worn entirely behind the ear, freeing recipients from long cables and extra components. How amazing I felt !! I didn’t have to worry anything hanging behind my ear and back. I was so happy when I received it.

The speech processor also has programs to change when not comfortable. It also has volume adjustment. This processor is also adjusted by the audiologist like with the powerful bodyworn speech processor. Same testing.

Wishing few appointments adjusting the sounds from the audiologist’s computer; the sounds gets better. You get used to the sounds. You would know if the sounds does not sound good. The audiologist would easily set up another appointment to adjust it for you. Like I mentioned you get two or three programs. You get to pick the sound on each programs where ever you are!

From when I started wearing Cochlear Implant back in 2000; cochlear implant had three batteries and now my cochlear implant has 2 batteries. Technology is getting better and better each few years. I love my cochlear implant now than my first cochlear implant because now I have bluetooth connected to my phone. I get to sleep at night with music playing in my cochlear implant. How cool is that! I also hear phone ringing from my phone into my Bluetooth Cochlear Implant. When I make a call to an office I get to hear the receptionist and no one else around me can hear. Another reason to say how awesome Cochlear is improving for all of us Deaf people.

I suggest flash cards when you receive your Cochlear processor. Have someone say the word on the flash card while covering their mouth. You say what you think you heard. Practice this and it will help you improve your hearing. It’s what I did for months. It helped me a lot!

Iam now a mom of 3 kiddos. I’m so blessed to hear their voice. I’m so glad I got the Cochlear Implant before I had my babies.

I thank all of those people who donate their money to House Ear Clinic. Also to California Medical Department for covering the expenses of the Cochlear Implant.

Hearing again changed my life! I appreciate it more to hear because for a whole two months being deaf was not easy for me. I was so used to hearing all the time with family and friends.

Iam now teaching my kids to learn more American Sign Language 〓〓 They will be my future interpreters.

If you have any questions regarding to Cochlear Implant; don’t hesitate to email me at SoCalDeafMom@gmail.com

Please check out my website SoCalDeafMom.com

Cochlear Anatomy via Microdissection with Clinical Implications An Atlas【電子書籍】[ Charles G. Wright ]
|

This atlas focuses on selected aspects of cochlear anatomy as illustrated by material prepared by the microdissection technique**,** a three-dimensional perspective not possible with standard histological approaches. Although much of the material in this text was processed by the microdissection method, photomicrographs from conventionally cross sectioned inner ear tissues and scanning electron microscopy, are also presented. Taken together, these technical approaches provide different, complementary views of inner ear anatomy, and offer a more informative understanding than is possible with any of the methods used alone. While micrographs obtained from microdissected material appear sporadically in journal articles, there is no comprehensive collection of such images currently available. The illustrations assembled in this atlas are complementary to the more traditional histologic images and drawings available in standard texts, and aide in understanding the intricate anatomy of the cochlea.

Cochlear Anatomy via Microdissection with Clinical Implications will be a useful resource for otolaryngologists, anatomists, audiologists, and neuroscientists. Those engaged in cochlear implantation, physicians and researchers in the growing fields of implantable hearing aids and drug delivery to the inner ear, and members of industry involved in designing, manufacturing, and marketing implantable hearing aids will also find this atlas of great value.
