<p>Throughout history, emerging and reemerging infectious disease have plagued human populations. From the earliest recorded epidemics of bubonic plague and smallpox more than 2,500 years ago to the deadly 1918 pandemic of the Spanish Flu, infectious diseases have helped to shape human history. Despite significant advances in medical research and treatment in the past century, infectious diseases remain among the leading causes of death worldwide. These diseases are appearing in places they have never been seen before or were thought to have been eradicated, are spreading faster and more frequently, and are posing an increasing global health threat that will affect national and international security in both the near- and long-term future, even affecting the success of U.S. military missions and operations.</p> <p>Force health protection is an urgent priority for the Department of Defense (DOD), as increasing threats of natural outbreaks of infectious disease could seriously undermine mission readiness and success. U.S. national security might be impacted by military operations in regions with endemic and epidemic occurrences of infectious disease, where disease activity may prevent the successful completion of a mission or operation or may even result in infected soldiers carrying a contagious disease back to the United States. Changing military doctrine and tactics call for a fresh approach to force protection. Rapid deployment of military forces, revised distribution of medical resources in theater, improved body armor, and modern combat casualty care efforts are leading to different illnesses and injuries from those seen in previous conflicts. Emerging infectious diseases in the context of urban warfare, low-intensity conflict, and the rapid movement of service members and civilians, have made a new approach to military medical support imperative. Despite improvements in combat-injury protection and wound management, disturbing trends are developing in infectious diseases in military forces.</p> <p>The purpose of this paper is to review important lessons that have been learned in the past, and to revisit the older but proven principles of force protection that are in danger of being forgotten in today's technology-focused military environment. The successful management of the Cuban Missile Crisis of 1962 set an unfortunate precedent for crisis management and national security crisis decision-making that persists into the contemporary security environment. The Cold War norm, roughly translated, meant that crisis management equaled crisis mitigation. The problem today is that the security environment has changed. Crisis management in the new environment should consider the use of crisis conditions to further the nation's security interests. The US has implemented formal changes to the structure of national security decision-making. Through the Goldwater-Nichols Department of Defense Reorganization Act of 1986 (GNA), the Congress sought to reorganize the defense establishment to provide for better military advice to civilian leaders and also to enable better operational execution during complex and fast-moving national security contingencies. The GNA expanded the authority of the regional combatant commanders (CCDR) and charged the CCDRs with the maintenance of security in their region. Finally, the informal roles and relationships between the civilian and uniformed leadership that change with new presidential administrations often influence the decision-making process and policy formulation. An initial examination of the existing crisis and crisis management definitions made it possible to project the new requirements for the contemporary security environment. Subsequent analysis of the CCDR's formal role in the decision-making process and a critical review of three recent military interventions (Operations DESERT SHIELD/DESERT STORM, ALLIED FORCE, and ENDURING FREEDOM) enabled a comparison of actual crisis behavior with the predefined crisis management norms and the evolving role of the CCDR. The comparison revealed that capabilities were not used and policy matters were overlooked because the CCDR either did not engage effectively or his advice was not considered. Nevertheless, the regional commander remained responsible for long-term security achievement of US objectives in his geographical region. The final analysis revealed that the politicians acted as if there were no changes to the environment or the roles of the uniformed military and the CCDRs did not fully understand their new role as a participant in the policy formulation process. The combined failures of the national security leadership allowed conflict conditions to persist long after the crisis response.</p> <p>The national security decision-making process ought to pay closer attention to the military advice of the regional CCDRs. Interagency planning and management efforts must include military advice in the decision-making process and/or policy formulation. CCDRs must be more aware of political considerations and willing to take part in the political discussion. Finally, the study suggested that formal changes in the decision-making process will not change the informal roles or relationships of the major players. While the U.S. Navy is proficient in delivering fuel and other materiel via underway replenishment, it also depends heavily upon a network of airports and seaports. The Combat Logistics Force, operating from these facilities, carries this materiel and moves it the last tactical mile; however, the Mark 41 Vertical Launch System (VLS) represents a critical vulnerability as it can only be reloaded while a ship is in port. Additionally, the Navy relies heavily upon access to port facilities that are often in range of potential adversaries possessing anti-access and area denial weaponry. Protecting this infrastructure and sustaining naval operations requires the cooperation of the other Services to provide air defense, force protection, and just-in-time delivery of munitions via inter-theater air transport. A review of naval operations in the 20th Century reveals operational insights and specific requirements for addressing MK 41 VLS replenishment in austere ports and anchorages. Several other near peer adversaries possess similar Anti-Access/Area Denial (A2/AD) capabilities to contest United States (U.S.) military operations. A Navy carrier strike group has the ability to defeat an initial attack but a significant expenditure of missiles, fired from the MK 41 Vertical Launch System (VLS), would diminish its capacity to withstand repeated attacks. The MK 41 VLS limiting factor is its reloading process, which requires the ship to be stable with minimal pitch, roll, and yaw. These conditions rarely occur in the open ocean. Lacking the ability to replenish VLS munitions at sea, a Navy carrier strike group would have to return to port in order to rearm its VLS missiles. During a conflict, the port in which rearming is conducted would require protection from adversary attack. In the event of conflict with a near peer adversary possessing weapons capable of striking the reloading port, the Navy may not be able to conduct VLS reloading at preferred port facilities due to threat of attack or actual damage to key facilities. The Navy may have to withdraw to another port outside the range of likely enemy attack in order to rearm its VLS equipped ships. Unfortunately, withdrawing from the battlespace risks ceding initiative to the adversary, which will slow the tempo of combat operations and present risk for the Combatant Commander.
