Yellow fever was a proximate cause of the American invasion of Cuba in 1898, and its control enabled a continued U.S. presence on the island and in the rest of the Caribbean. Had it not been for influenza, German Gen. Erich Ludendorff might well have succeeded in his offensive in the closing years of World War I. <p>All infectious diseases may spread in consequence of war and develop into epidemics of varying extent. In the next chapter we shall see how the wars at the end of the fifteenth century favoured the spread of an epidemic of syphilis. In the Union Army, during the American Civil War of 18615, both measles and typhoid fever were very widespread, and together they were the cause of 4,246 deaths, or about 1・75 per cent of the total enlistment. Scarlet fever, influenza, yellow fever, relapsing fever, and malaria (if the war is waged in countries where this disease is endemicーespecially in the Lower Danube region, in the Netherlands, Spain, and Italy) have also played an important r?le in many wars. But we give the name ‘war pestilences’ only to those infectious diseases which in the course of centuries have usually followed at the heels of belligerent armies, such as typhus fever, bubonic plague, cholera, typhoid fever, dysentery, and small-pox; we may also include here scurvy, the etiology of which has not yet been definitely determined. Typhus fever (spotted fever, exanthematic typhusーcalled in France and England simply typhus, in Spain tabardilloーformerly called contagious typhus, hunger typhus, camp fever, and Hungarian fever) is an acute infectious disease of cyclic recurrence, which resembles typhoid fever only in name. From the eighth to the tenth day after infection, often somewhat sooner or later, it begins with a chill, accompanied by nausea, vomiting, violent headache, and psychic depression. In the first few days the patient’s temperature rises rapidly, and on the fourth or fifth day a rash in the form of dull-red spots, as large as a pea, breaks out over the entire body. These spots gradually grow larger, and after two or three days, through the appearance of very small haemorrhages, change into petechiae. The apathy of which the patient first gave evidence now gives way to wild delirium. At the end of the second week the temperature falls rapidly, and in one or two days becomes normal; often, however, the fall of temperature takes from six to eight days. The duration of the entire disease, accordingly, is from two to two and a half weeks. Death usually occurs at the crisis of the diseaseーfrom the tenth to the twelfth dayーrarely between the sixth and ninth days or after the twelfth. The danger of the disease varies greatly in different epidemics; statements regarding this point diverge according as we refer to the statistical records of hospitals or to the private practice of physicians. With the latter the number of deaths is smaller, since persons suffering from the disease in mild form less often go to the hospitals. Epidemics in which a quarter of the patients, and even more, have succumbed have frequently occurred, especially in war times, during famines, &c. The cause (infective agent) of typhus fever is not known; according to recent investigations it is spread by vermin; Ricketts and others have fixed responsibility for it upon the body louse. The infection is communicated from man to man, and very often it is contracted from the clothes, linen, and other effects of typhus patients. Recovery from the disease usually renders a person immune against a second attack. Whereas many late twentieth century scholars blamed the catastrophic decline of postconquest native populations on the introduction of previously unknown infections from the Old World, Alchon argues that the experiences of native peoples in the New World closely resembled those of other human populations. Exposure to lethal new infections resulted in rates of morbidity and mortality among native Americans comparable to those found among Old World populations.</p> <p>Why then did native American populations decline by 75 to 90 percent in the century following contact with Europeans? Why did these populations fail to recover, in contrast to those of Africa, Asia, and Europe? Alchon points to the practices of European colonialism. <p>The Middle Age of European history has no naturally fixed beginning or ending. The period of Antiquity may be taken as concluded by the fourth Christian century, or by the fifth or by the sixth; the Modern period may be made to commence in the fourteenth, or in the fifteenth or in the sixteenth. The historian Hallam includes a thousand years in the medieval period, from the invasion of France by Clovis to the invasion of Italy by Charles VIII. in 1494. We begin, he says, in darkness and calamity, and we break off as the morning breathes upon us and the twilight reddens into the lustre of day. To the epidemiologist the medieval period is rounded more definitely. At the one end comes the great plague in the reign of Justinian, and at the other end the Black Death. Those are the two greatest pestilences in recorded history; each has no parallel except in the other. They were in the march of events, and should not be fixed upon as doing more than their share in shaping the course of history. But no single thing stands out more clearly as the stroke of fate in bringing the ancient civilization to an end than the vast depopulation and solitude made by the plague which came with the corn-ships from Egypt to Byzantium in the year 543; and nothing marks so definitely the emergence of Europe from the middle period of stagnation as the other depopulation and social upheaval made by the plague which came in the overland track of Genoese and Venetian traders from China in the year 1347. While many other influences were in the air to determine the oncoming and the offgoing of the middle darkness, those two world-wide pestilences were singular in their respective effects: of the one, we may say that it turned the key of the medieval prison-house; and of the other, that it unlocked the door after eight hundred years. The Black Death and its after-effects will occupy a large part of this work, so that what has just been said of it will not stand as a bare assertion. But the plague in the reign of Justinian hardly touches British history, and must be left with a brief reference. Gibbon was not insensible of the part that it played in the great drama of his history. “There was,” he says, “a visible decrease of the human species, which has never been repaired in some of the fairest countries of the globe.” After vainly trying to construe the arithmetic of Procopius, who was a witness of the calamity at Byzantium, he agrees to strike off one or more ciphers, and adopts as an estimate “not wholly inadmissible,” a mortality of one hundred millions. The effects of that depopulation, in part due to war, are not followed in the history. So far as Gibbon’s method could go, the plague came for him into the same group of phenomena as comets and earthquakes; it was part of the stage scenery amidst which the drama of emperors, pontiffs, generals, eunuchs, Theodoras, and adventurers proceeded.
