In Abh?ngigkeit von Dauer und St?rke der Noxe oder des Traumas sowie der Immunlage des Patienten kann sich dieser Zustand in systemischem Ausma? manifestieren. Dieser Status der 'Ganzk?rperentz?ndung' ist ein kongruentes klinisches Syndrom verschiedenster ?tiologien. So k?nnen ausgedehnte Isch?mien, h?morrhagischer Schock, Pankreatitis, Polytrauma und Verbrennung ebenso wie verschiedene Infektionen (bakteriell, viral, parasit?r) ab einem bestimmten Ausma? zu einem ?hnlichen oder sogar identischen klinischen Bild f?hren. Dieser stereotype Symptomenkomplex geht von einer exzessiven und inad?quaten Produktion von Entz?ndungsmediatoren durch aktivierte Leukozyten aus und wird nach neuester, international g?ltiger Definition, unabh?ngig von der Ursache, als SIRS (systemic inflammatory response syndrome) bezeichnet [Bone et al. 1992]. Die bisherige Uneinigkeit ?ber das Krankheitsbild 'Sepsis' und seine Einschlu?kriterien dr?ckt sich unter anderem in international ?u?erst divergierenden Angaben zu Inzidenz und Mortalit?t aus. Daher wurde eine neue und differenzierte Begriffsbestimmung notwendig. Die Sepsis versteht sich nach dieser Definition der American Thoracic Society und der Society of Critical Care Medicine als Systemantwort auf eine Infektion und ist Teilbereich des SIRS. Einschlu?kriterien sind Hypo- oder Hyperthermie, Tachykardie, Hyperventilation und Leukozytose/Leukopenie, wobei zwei oder mehr Merkmale zutreffen m?ssen. Die Sepsis dient exemplarisch vielen experimentellen Modellen zur Erforschung der wahrscheinlich gemeinsamen Pathogenese und Pathophysiologie des SIRS.
