Knowledge Sharing in Chinese Hospitals Identifying Sharing Barriers in Traditional Chinese and Western Medicine Collaboration【電子書籍】[ Lihong Zhou ] 楽天Kobo電子書籍ストア	＜p＞This book aims to identify, understand and qualify barriers to the patient-centred knowledge sharing (KS) in interprofessional practice of Traditional Chinese Medicine (TCM) and Western Medicine (WM) healthcare professionals in Chinese hospitals. This collaboration is particularly crucial and unique to China since, contrary to Western practice, these two types of professionals actually work together complimentary in the same hospital. This study adopted a Grounded Theory approach as the overarching methodology to guide the analysis of the data collected in a single case-study design. A public hospital in central China was selected as the case-study site, at which 49 informants were interviewed by using semi-structured and evolving interview scripts. The research findings point to five categories of KS barriers: contextual influences, hospital management, philosophical divergence, Chinese healthcare education and interprofessional training. Further conceptualising the research findings, it is identified that KS is mostly prevented by philosophical and professional tensions between the two medical communities. Therefore, to improve KS and reduce the effects of the identified barriers, efforts should be made targeted at resolving both types of tensions. In Germany, there is a dual hospital funding. The costs of operation are beared by payments of health insurance funds. Investment costs for new buildings or the replacement of capital goods are payed by the federal states. However, these allowances of investment are on the decrease for years, which leads to investment backlogs in hospital (Augurzky et al. 2009, 93). This implies that hospitals are supposed to invest, but their funds are too small to do so. In the long run, the economic efficiency suffers because it cannot compete with other hospitals regarding the technological progress (Augurzky et al. 2009, 13). The introduction of DRGs [Diagnosis Related Groups], the basis of calculation for hospitals, lead to an increasing pressure of working economically. In the old system, every day of a patient's stay in the hospital was refunded based on same-day hospital and nursing charges. In the new system, only occupant days within a predetermined period of hospitalization. The preterm discharge or a discharge exceeding the period of hospitalization results in discounts in payments, which often do not allow cost recovery (Eveslage 2006, 37-39). Accordingly, hospitals are under pressure to treat their patients fast and discharge them within the preset period. This requires efficient and economical operations. An additional burden is the growing competition in the sector of ambulatory care. As a result of the strong medical progress, more and more operations, which were formerly bound to be performed in hospital, can nowadays be done ambulant. Another innovation in the German health care system are medical service centers [Medizinische Versorgungszentren]. They will soon be capable to take over the primary health care in rural areas and replace major hospitals there, because they are able to work more economic (Augurzky et al. 2009, 162). One of the factors that contribute to the provision of excellent services is the presence of competent managers. In this book, we report the outcome of a study aimed to determine the level of service excellence among managers and the factors influencing it in five Military Hospitals under the Ministry of Defence, Malaysia. A cross sectional study was conducted using self-administered questionnaires distributed to Military and Non-Military managers with different demographic backgrounds and service characteristics. The questionnaires consist of six components: the background of the respondents; the level of service excellence; skills; knowledge of the Blue Ocean Strategy (BOS), leadership style, and organisational compliance. By using the Structural Equation Model, findings of the study showed that all independent variables, i.e., skills, knowledge on BOS, leadership style and organisational compliance are significantly related to service excellence. Using Stepwise Multiple Linear Regression, it was discovered that managers in the lower grades, as well as Non-Military managers, demonstrated significant service excellence. In conclusion, the service excellence of managers in Military Hospitals is strongly influenced by their competencies.
