<p>Recent advances in technology have greatly impacted upon the practice of nuclear medicine in general, and the approach to hepatobiliary diseases in particular. Empha sis is now placed more on non-invasive functional imaging with quantification, a feature unique to nuclear medicine and not available from any other competing diag nostic imaging modality. By providing a measure of severity of disease, quantification aids not only in timing of therapy but also for testing whether or not the chosen therapy has achieved the intended goals and objectives. This is the first textbook of its kind in the new millennium on nuclear hepatology to fully integrate quantitative physiology with morphology in the diagnosis of hepatobiliary diseases. The conception of nuclear hepatology occurred in the late 1940s with the introduc tion of radiocolloids, whose rate of clearance from blood was used as a measure of hepatic blood flow. <p>Since the publication of the first edition of Nuclear Hepatology almost 10 years ago, new advances have taken place, both in our understanding of liver pathophysiology and various imaging modalities. A unique feature of imaging of physiology is that it enables quanti- cation. Since liver physiology is complex, quantification has been a challenge. Many p- lications in the past have often included quantification based on home-made software not available for others. Thus, the comparison of results from one center to another becomes difficult, if not impossible. The second edition of Nuclear Hepatology addresses these issues. Sophisticated software for liver and gallbladder function has been tested and va- dated through many of our previous publications. Now that software is FDA approved and available for others through a commercial company, it is hoped that future publications of imaging of liver and gallbladder physiology would routinely include quantification. Although the function of the hepatocyte is complex, it can be broadly divided into two main categories, substrate uptake at the basolateral border and intracellular transit prior to excretion into the canaliculi. The earliest manifestation of hepatocellular injury occurs in the form of prolongation of the intracellular transit time, which occurs long before the disruption of the basolateral border with subsequent rupture and death. <p>When one deals with cancer, the hepatobiliary malignancies present a chal lenge to the oncologists that can be characterized as a series of unsolved clinical and biological dilemmas. Liver metastases from colorectal and other gastrointestinal malignancies, hepatocellular carcinoma, cholangiocar cinoma, and gall bladder cancer present an array of problems but have two features in common. These are high morbidity and mortality with an overall poor result from treatment. Why is it that hepatobiliary cancer carries with it such a dismal prognosis? First of all, these diseases present, for the most part, in an advanced state. To this point in time the oncologist has had no help from early diagnosis or screening. Only the occasional patient followed by ultrasound or a tumor marker has the disease diagnosed in an asymptomatic state. By the time these diseases become symptomatic, curative treatment options have usually disappeared. Evolution has placed the liver in a protected position in order to avoid injury to the soft parenchyma. As with many other internal organs, the nerve supply is extremely limited. <p>Surgery an essentially manual exercise in its early days, has progressively grown richer in complementary techniques helping the surgeon to perfect his movements and increase efficiency. This is particularly the case with digestive surgery, which has been thoroughly transformed by radiology, endoscopy and extemporaneous examinations effected during surgical intervention. Such methods make it possible for surgeons to develop subtle nuances in operative techniques and to specify indi cations more and more precisely. Ultrasonography must now be included among such techniques. It supplements them and can sometimes even replace them. For these reasons, this work devoted to the use of ultrasound by the surgeon during surgical intervention is of great interest. Two general conditions had to be met before ultrasonography could be em ployed in abdominal surgery: the method and apparatus had to be adapted to its surgical utilization, and the surgeon had to adapt to a nonsurgical technique. The first condition has been fulfilled for all purposes. Intraoperative sonograms can be generated at a very high rate and are proving to be more and more useful. The miniaturization of probes permits their application almost everywhere with opti mal results.
