<p>There is now evidence that irreversible brain damage accumulates very early in the course of multiple sclerosis. This book reviews the main neurobiological, magnetic resonance imaging, and clinical aspects of the early phases of the dis ease. Mechanisms ofirreversible axonal damage and the role played by the inter action of glia and the axon are highlighted. In contrast to what was believed for a long time, the sufficient availability of oligodendrocyte precursor cells to promote remyelination in acute lesions has now been demonstrated. For reasons not understood, this remyelination process fails or does not start, particularly in the chronic stages ofthe disease. These findings emphasize the importance of the "milieu" changes induced by an inflammatory process in limiting remyelination. However, first indications are that part of this inflammatory process may have a neuroprotective effect. Pathological studies in multiple sclerosis have now clearly demonstrated that destructive processes may be followed by recovery phases in such a way that myelin may be morphologically and functionally reconstituted. These findings provide the rationale for early treatment and emphasize the importance of clinical trials in early multiple sclerosis. Thus, there is an urgent need to expand our knowledge to find suitable therapeutic strategies to enhance neurorepair processes in such diseases.This volume presents neuroprotection and novel therapeutic strategies developed in the last 5 years by 12 world leaders in the field. The term neuroprotection means rescuing neuronal and non-neuronal cells together. The cerebral endothelium that constitutes the anatomical and physiological site of the blood-brain barrier (BBB) is one of the most important non-neural cells in the CNS. Any distortion of the BBB leads to brain diseases and restoration of the barrier results in neuroprotection. Thus, the BBB appears to be the "gateway" for neurological diseases and neurorepair. However, to treat brain tumors or infarcts, new therapeutic strategies are needed to enhance brain drug delivery using nanotechnology. In addition, apart from conventional drugs, restoration of BBB function could also be achieved by means of antibodies directed against specific proteins, neurotransmitters or exogenous supplement of neurotrophic factors. <p>Cerebral preconditioning is a phenomenon wherein a mild insult or stress induces cellular and tissue adaptation or tolerance to a later, severe injury, therefore reflecting the efficacy of endogenous mechanisms of cerebrovascular protection. Initially identified for rapid cardiac protection, preconditioning has expanded to all aspects of CNS protection from ischemia, trauma and potentially neurodegeneration. Many different stimuli or stressors have been identified as preconditioning agents, suggesting a downstream convergence of mechanisms and underscoring the potential for translational application of preconditioning in the clinic. Moreover, the fundamental mechanisms responsible for preconditioning-induced tolerance will help in the design novel pharmacological approaches for neuroprotection. While stroke and many other brain injuries are not predictable, in some populations (e.g., metabolic syndrome, patients undergoing carotid endarterectomy, aneurysm clipping, or with recent TIAs) the risk for stroke is identifiable and significant, and preconditioning may represent a useful strategy for neuroprotection. For unpredictable injuries, post-conditioning the brain ? or inducing endogenous protective mechanisms after the initial injury ? can also abrogate the extent of injury.
