<p>In recent years, the progress made in the prevention of mortality and morbidity caused by communicable diseases and malnutrition has changed the disease spectrum in both developed and, particularly developing countries. As a result, noncommunicable diseases, includ ing genetic disorders, have achieved considerable importance in public health. Furthermore, it is now evident that inherited predisposition is important in a number of common diseases that occur in later life, such as atherosclerosis, coronary heart disease, hypertension, diabetes mellitus, and in some rheumatic, oncological, and mental illnesses that appear at an early stage and develop into severe handicaps in predisposed people. Rapid advances in gene mapping concerned with international human genome research make it almost certain that the use of new genetic knowledge will dramatically increase the requirement for genetic approaches in the control of a wide spectrum of diseases, and will provide possibilities for their prevention and treatment in the form of changes in lifestyle, diet modification, periodic check-ups, or the administration of gene therapy. It appears that one of the main problems in delivering genetics services is the difficulty involved in informing the health profession and the community of the real sig nificance of genetic problems. Increasing life expectancy in South Asia is resulting in a demographic transition that can under the right circumstances yield dividends through more favorable dependency ratios for a time. With aging the disease burden shifts toward noncommunicable diseases (NCDs) which can threaten healthy aging. However securing the gains expected from the demographic dividendwhere developing countries working and nondependent population increases and per capita income thus rises is both achievable and affordable through efficiently tacking NCDs with prevention and control efforts. This book looks primarily at cardiovascular disease (CVD) and tobacco use since they account for a disproportionate amount of the NCD burdenthe focus is strategic rather than comprehensive. The goal of this book is to encourage countries to develop adopt and implement effective and timely country and where appropriate regional responses that reduce both population-level risk factors and the NCD burden. The work develops (i) an NCD burden and risk factor profile for all countries and the region as a whole; (ii) a rationale for public policy and action for NCDs; (iii) a framework to guide the formulation of public policies and strategies for NCDs; (iv) a country profile including capacity and ongoing NCD activities as well as policy options and actions for NCDs that will help stimulate policy dialogue within and among countries; and (v) a regional strategy for NCD prevention and control where regional collaboration offers added value. <p>The health workforce must be able to cope with shifts in the pattern of causes of death and disease that are being seen worldwideーparticularly with the rise in noncommunicable diseases (NCDs). This requires health personnel to have a set of skills and competencies that can adapt to meet the population’s growing and changing health needs.This report examines the status of the nurse workforce in the Eastern Caribbean, assessing how best to strengthen its capacity to respond to the growing burden of NCDs. The report is based on four Eastern Caribbean case studies conducted in Dominica, Grenada, St. Lucia, and St. Vincent and the Grenadines.The case studies showed that, although overall nurse staffing levels seem to be sufficient across the four countries in terms of numbers, there are nurse shortages at hospitals and health clinics, as well as in the availability of specialists. Better management of the nurse workforce is needed to ensure adequate coverage at the hospitals and clinics is maintained. In terms of specialists, while the quality of education is generally reported to be good, there are barriers to accessing specialized training which include the high-cost of seeking specialized training due to non-existing or very limited local options and the lack of built-in incentives for completing additional education.To help address the capacity constraints faced by the nurse workforce in the Eastern Caribbean to respond to NCDs, the report generated knowledge in support of two critical areas: (i) provide an understanding of the educational and training opportunities available to nurses to strengthen their capacity to meet the NCD challenges, and (ii) assess whether there is a supportive policy environment in place for nurses to play a role in addressing NCDs.
