<p>Iodine-131, a radioisotope with significant health implications that can be generated by nuclear power plant accidents and meltdowns, is fully covered in this authoritative, 600 page collection of official documents with details about the health and medical impacts of this dangerous fission product. Traces of iodine-131 were detected coming from the damaged reactors at the Japanese Fukushima Power Station in March 2011. A complete toxicological profile of iodine-131 from the Agency for Toxic Substances and Disease Registry (ATSDR) is included. There is also full coverage of a drug, potassium iodide, that is used to block the uptake of thyroid radioiodine, reducing the risk of thyroid cancer in populations at risk.</p> <p>A large amount of epidemiological literature exists on the health outcomes in populations that were exposed to environmental releases of radioiodine. These include releases from explosions of nuclear bombs such as the Marshall Islands BRAVO test, the largest U.S. detonation (15 megatons), and from the Nevada Test Site; releases from nuclear fuel production facilities such as the Hanford Nuclear Site; and accidental releases from nuclear power plants such as the Chernobyl explosion and fire. In general, releases of these types result in mixed exposures to a variety of radioisotopes and to radiation doses from both external and internal exposure. However, doses from radioiodine that are significant to health derive largely from internal exposure as a result of uptake of relatively short-lived radioiodine isotopes into the thyroid gland. Thus, effects on the thyroid attributable to radioiodine that were subsequently observed, in some cases, years after the event, derived from exposures to the relatively high levels of radioiodine found immediately after the event, rather than from sustained exposures.</p> <p>Potassium iodide is a salt, similar to table salt. Its chemical symbol is KI. It is routinely added to table salt to make it "iodized." Potassium iodide, if taken in time and at the appropriate dosage, blocks the thyroid gland's uptake of radioactive iodine and thus could reduce the risk of thyroid cancers and other diseases that might otherwise be caused by exposure to radioactive iodine that could be dispersed in a severe nuclear accident. Potassium iodide is a special kind of protective measure in that it offers very specialized protection. Potassium iodide protects the thyroid gland against internal uptake of radioiodines that may be released in the unlikely event of a nuclear reactor accident. The purpose of radiological emergency preparedness is to protect people from the effects of radiation exposure after an accident at a nuclear power plant. Evacuation is the most effective protective measure in the event of a radiological emergency because it protects the whole body (including the thyroid gland and other organs) from all radionuclides and all exposure pathways. Spent nuclear fuel reprocessing plant wastes may introduce small amounts to the environment. However, the U.S. does not currently reprocess spent nuclear fuel.</p> <p>Since the 1960s, Prussian blue has been used to treat people who have been internally contaminated with radioactive cesium (mainly Cs-137) and nonradioactive thallium (once an ingredient in rat poisons). Doctors can prescribe Prussian blue at any point after they have determined that a person who is internally contaminated would benefit from treatment. Prussian blue will help speed up the removal of cesium and thallium from the body. Prussian blue traps radioactive cesium and thallium (mainly Tl-201) in the intestines and keeps them from being re-absorbed by the body. The radioactive materials then move through the intestines and are excreted (passed) in bowel movements. Prussian blue reduces the biological half-life1 of cesium from about 110 days to about 30 days. Prussian blue reduces the biological half-life of thallium from about 8 days to about 3 days. Since they have no moving parts that can fail or wear out, RTGs have historically been viewed as a highly reliable power option. Thermocouples have been used in RTGs for a total combined time of over 300 years, and a not a single thermocouple has ever ceased producing power. Thermocouples are common in everyday items that must monitor or regulate their temperature, such as air conditioners, refrigerators and medical thermometers. The principle of a thermocouple involves two plates, each made of a different metal that conducts electricity. Joining these two plates to form a closed electrical circuit while keeping the two junctions at different temperatures produces an electric current. Each of these pairs of junctions forms an individual thermocouple. Even as the atomic bomb became the focus of Cold War anxiety, radioisotopes represented the government’s efforts to harness the power of the atom for peaceーadvancing medicine, domestic energy, and foreign relations.</p> <p>In <em>Life Atomic</em>, Angela N. H. Creager tells the story of how these radioisotopes, which were simultaneously scientific tools and political icons, transformed biomedicine and ecology. Government-produced radioisotopes provided physicians with new tools for diagnosis and therapy, specifically cancer therapy, and enabled biologists to trace molecular transformations. Yet the government’s attempt to present radioisotopes as marvelous dividends of the atomic age was undercut in the 1950s by the fallout debates, as scientists and citizens recognized the hazards of low-level radiation. Creager reveals that growing consciousness of the danger of radioactivity did not reduce the demand for radioisotopes at hospitals and laboratories, but it did change their popular representation from a therapeutic agent to an environmental poison.
