Aplastic Anemia and Other Bone Marrow Failure Syndromes【

During the past decade, there have been numerous direct and indirect scientific contributions to both the etiology and therapy of aplastic anemia and related bone marrow failure syndromes. Clinical observations, such as autologous bone marrow recovery after conditioning with immunosup pressive agents for bone marrow transplantation; failure to achieve en graftment in some identical twins without prior immunosuppressive ther apy; and hematologic response to immunosuppressive agents, have led to the concept of immune-mediated etiology of acquired aplastic anemia. Such a concept was further strengthened by laboratory findings, implicat ing the role of activated cytotoxic T lymphocytes and abnormal produc tion of inhibitory lymphokines. The immunologic mechanisms may also apply to the idiosyncratic bone marrow aplasias associated with drugs, toxic chemicals, and viruses. These agents may alter normal cellular recog nition sites by interacting with cellular components and result in loss of self tolerance. Immunologic mechanisms have long been advocated in many other organ failures, and the hemopoietic organ is no exception. It is of interest that parallel clinical and laboratory investigations in juvenile diabetes mellitus type I and in rodent models of this disease have yielded results compatible with the same pathogenic mechanisms. The infiltration of pancreatic islets by activated T lymphocytes, functional and morphological alterations of islet cells upon incubation with lymphokines such as gamma interferon and tumor necrosis factor, and clinical response to cyclosporine are a few examples.

Types of Anemia, A Simple Guide To The Condition, Diagnosis, Treatment And Related Conditions【電子書籍】[ Kenneth Kee ]

This book describes Types of Anemia, Diagnosis and Treatment and Related Diseases
Anemia is a disorder that develops when the blood does not have enough healthy red blood cells or hemoglobin.
Hemoglobin is an essential part of red blood cells and binds oxygen.
If the patient has too few or abnormal red blood cells, or the hemoglobin is abnormal or low, the cells in the body will not get enough oxygen.
Symptoms of anemia like fatigue happen because organs are not getting what they need to function properly.
Some types of anemia are hereditary and infants may be involved from the time of birth.
Women in the childbearing years are mostly vulnerable to iron-deficiency anemia because of the blood loss from menstruation and the higher blood supply demands during pregnancy.
Older adults also may have a greater danger of developing anemia because of poor diet.
Iron-deficiency anemia, the most frequent type, is very treatable with diet changes and iron supplements.
There are more than 400 forms of anemia, which are classified into 3 groups:
1.Anemia produced by blood loss
Red blood cells can be lost through bleeding, which often can occur slowly over a long period undetected.
a.Gastrointestinal disorders such as ulcers, hemorrhoids, gastritis and cancer
b.Use of non-steroidal anti-inflammatory drugs such as aspirin, which can produce ulcers and gastritis
c.Menstruation, if menstrual bleeding is excessive
2.Anemia produced by decreased or abnormal red blood cell production
With this form of anemia, the body may form too few blood cells or the blood cells may not function correctly
Red blood cells may be faulty or reduced due to abnormal red blood cells or a lack of minerals and vitamins needed for red blood cells to work properly:
a.Sickle cell anemia
b.Iron-deficiency anemia
c.Vitamin B12 deficiency
d.Bone marrow and stem cell problems
Aplastic anemia occurs when there is a marked reduction in the number of stem cells or absence of these cells.
Aplastic anemia can be inherited, can occur without apparent cause, or can happen when the bone marrow is injured by medicines, radiation, chemotherapy, or infection
Thalassemia is an inherited disorder that occur when the red cells cannot mature and grow properly
This disorder can vary in severity from mild to life threatening; the most severe form is called Cooley's anemia.
Lead exposure is toxic to the bone marrow, producing fewer red blood cells.
Lead poisoning happens in adults from work-related exposure and in children who eat paint chips.
Anemia linked with other disorders normally happens when there are too few hormones for red blood cell production:
a.Advanced kidney disease
b.Hypothyroidism
c.Other chronic diseases, such as cancer, infection, lupus, diabetes, and rheumatoid arthritis
d.Old age
3.Anemia produced by destruction of red blood cells
When red blood cells are fragile and cannot resist the routine stress, they may burst prematurely, causing hemolytic anemia:
a.Inherited conditions, such as sickle cell anemia and thalassemia
b.Stressors such as infections, drugs, snake or spider venom, or certain foods
c.Toxins from advanced liver or kidney disease
d.Improper attack by the immune system
e.Vascular grafts, prosthetic heart valves, tumors, severe burns, exposure to certain chemicals, severe hypertension, and clotting disorders
Symptoms of anemia can be anything from shortness of breath and dizziness to headaches, pale skin, chest pain, fast heart rate, and cold hands and feet.
Treatments depend on the cause of anemia with supplement of Vitamin B12, folate and minerals.
A way to treat anemia is with a blood transfusion
A stem cell transplant also treats most anemia

TABLE OF CONTENT
Introduction
Chapter 1 Types of Anemia
Chapter 2 Treatment
Chapter 3 Iron Deficiency Anemia
Chapter 4 Hemolytic Anemia
Chapter 5 Pernicious Anemia
Chapter 6 Megaloblastic Anemia
Chapter 7 Aplastic Anemia
Chapter 8 Rare Anemias
Epilogue

This case report describes the poor outcome of a female patient who underwent elective nephrectomy. Unfortunately, the patient was suffering from undiagnosed preoperative platelet dysfunction and anemia, although blood count and clotting were almost within normal ranges two weeks earlier at the day of preoperative assessment for anesthesia. In this respect, intensive preoperative evaluation of the coagulation system embedded in a comprehensive patient blood management (PBM) concept might have the potential to detect preoperative coagulopathy and anemia and thereby could improve clinical outcome.

About the Patient Blood Management Case Report Series

Patient Blood Management (PBM) is a multidisciplinary, evidence-based approach to improve perioperative patient care. The concept includes preoperative screening, diagnosis and treatment of anemia and hemostasis, adequate blood product utilization and blood-sparing techniques.
Sickle cell anemia is the most common form of sickle cell disease (SCD).
SCD is a serious disorder in which the body makes sickle-shaped red blood cells.
Sickle-shaped means that the red blood cells are shaped like a crescent.
Sickle cells contain abnormal hemoglobin called sickle hemoglobin or hemoglobin S.
Sickle hemoglobin causes the cells to develop a sickle or crescent, shape.
Sickle cells are stiff and sticky.
They tend to block blood flow in the blood vessels of the limbs and organs.
Blocked blood flow can cause pain and organ damage.
It can also raise the risk for infection.
Anemia is a condition in which the blood has a lower than normal number of red blood cells and lower hemoglobin.
In sickle cell anemia, the abnormal sickle cells usually die after only about 10 to 20 days unlike the 120 days of normal red blood cell.
The bone marrow cannot make new red blood cells fast enough to replace the dying ones.
Sickle cell anemia is an inherited, lifelong disease.
The person inherits two genes for sickle hemoglobin one from each parent.
People who inherit a sickle hemoglobin gene from one parent and a normal gene from the other parent have a condition called sickle cell trait.
Sickle cell trait is different than sickle cell anemia.
People who have sickle cell trait do not have the disease.
Like people who have sickle cell anemia, people who have sickle cell trait can pass the sickle hemoglobin gene to their children.
Sickle cell anemia is present at birth but many infants do not show any signs until after 4 months of age.
The most common signs and symptoms are linked to anemia and pain.
The most common symptom of anemia is fatigue (feeling tired or weak).
There is also:
a. Shortness of breath
b. Dizziness
c. Headaches
d. Coldness in the hands and feet
e. Paler than normal skin or mucous membranes (the tissue that lines the nose, mouth, and other organs and body cavities)
f. Jaundice
Sudden pain throughout the body is a common symptom of sickle cell anemia.
This pain is called a sickle cell crisis.
Sickle cell crises often affect the bones, lungs, abdomen, and joints.
These crises occur when sickled red blood cells block blood flow to the limbs and organs.
This can cause pain and organ damage.
Chronic pain often lasts for weeks or months and can be hard to bear and mentally draining
A simple blood test done at any time during a person's lifespan can detect whether he or she has sickle hemoglobin.
Doctors also can diagnose sickle cell disease before and after birth.
Sickle cell anemia has no widely available cure.
However treatments can help relieve symptoms and treat complications.
The goals of treating sickle cell anemia are to relieve pain; prevent infections, organ damage, and strokes; and control complications
Blood and marrow stem cell transplants may offer a cure for a small number of people who have sickle cell anemia.
Severe sickle cell anemia can be treated with a medicine called hydroxyurea.
This medicine prompts the body to make fetal hemoglobin
New medicine are:
Decitabine.
Adenosine A2a receptor agonists
TABLE OF CONTENT
Introduction
Chapter 1 Sickle Cell Anemia
Chapter 2 More Facts about Sickle Cell Anemia
Chapter 3 Treatment of Sickle Cell Anemia
Chapter 4 Hemolytic Anemia
Chapter 5 G6PD Deficiency
Chapter 6 Thalassemia
Epilogue

This groundbreaking book chronicles the history of sickle cell anemia in the United States, tracing its transformation from an "invisible" malady to a powerful, yet contested, cultural symbol of African American pain and suffering.

Set in Memphis, home of one of the nation's first sickle cell clinics, Dying in the City of the Blues reveals how the recognition, treatment, social understanding, and symbolism of the disease evolved in the twentieth century, shaped by the politics of race, region, health care, and biomedicine. Using medical journals, patients' accounts, black newspapers, blues lyrics, and many other sources, Keith Wailoo follows the disease and its sufferers from the early days of obscurity before sickle cell's "discovery" by Western medicine; through its rise to clinical, scientific, and social prominence in the 1950s; to its politicization in the 1970s and 1980s. Looking forward, he considers the consequences of managed care on the politics of disease in the twenty-first century.

A rich and multilayered narrative, Dying in the City of the Blues offers valuable new insight into the African American experience, the impact of race relations and ideologies on health care, and the politics of science, medicine, and disease.

Aplastic Anemia Pathophysiology and Approaches to Therapy【

Research on aplastic anaemia has until recently been limited to clinical description, morphology and epidemiology. New methods to culture haemopoietic cells, and advances in our knowledge of proliferation and differentiation in the haemopoietic cell system .opened a new area of scientific interest for this "prototype" of haemopoietic failure. In addition, bone marrow transplantation became not only a clinical method of treatment, but also a source of data useful for the discussion of pathophysiological models of aplastic anaemia. This situation prompted us to arrange an international con ference on aplastic anaemia, with particular emphasis on its patho physiology and the rationals of the current therapeutic approaches. This conference was held at Schloss Reisensburg from July 20-22, 1978 with the participation of both experimental and clinical scientists active in this field or in related areas of research. The proceedings of the symposion reflect the present knowledge as well as the many new questions which arose from the discussions. The editors are gratefully indebted to the participants of this meeting, to Gerlinde Trogele and all the co-workers of the Uni versity of Ulm engaged in preparation of this symposium and of this volume, and last not least to all sponsors who provided the financial basis for this scientific event.
