Avoiding Medical Errors One Hundred Rules to Help You Survive Mistakes by Doctors and Hospitals【電子書籍】[ Robert M. Fox ]

This book, written by a lawyer and a doctor explains to everyday readers ways in which they can avoid death and injury caused by medical mistakes. It may be shocking to learn that preventable errors by doctor and hospital personnel are a leading cause of death and injury in the United Statesーperhaps even exceeding the annual deaths caused by heart disease and cancer. But avoiding these mistakes is possible, and the rules found in this book will arm readers against the careless errors that lead to such deaths and injuries.

From hospitals to doctors’ offices, medical professionals are overwhelmed, overtired, even overworked and mistakes are sometimes unavoidable even with the best safety measures in place. A resident at the end of a 36-hour on-call stint may forget to wash her hands before performing a surgical procedure. A chart may be mismarked. Medications may be inaccurately listed. Test results may be inaccurately interpreted. But patients are in a position to help themselves and their medical caregivers to avoid these mistakes by taking more active and attentive part in their own healthcare. By being aware of the most common errors, patients can look for ways to ask questions, review information, even examine test results with a critical eye toward their own health and specific situations. Robert Fox and Chris Landon show them how.

The Catholic Church and Its Hospitals A Marriage Made in Heaven?【電子書籍】[ Patricia A Gabow ]

Do you know if your hospital is Catholic? Do you know what that means for your care and your community? The answers are most probably "No."

This book will answer these questions and pose many others in an expansive examination of the Catholic healthcare system in the United States. This well-researched volume delves deep into the origins, evolution, and the present-day implications of the Catholic healthcare system in the U.S. It traces Catholic healthcare's lineage from its biblical foundation to the role of courageous women religious in providing care to those in need, to the modern era of bishops' control over hospitals, doctors, and their clinical practice, determining the care that will be provided to millions of Americans.

In the U.S. in 2021, six of the 25 largest healthcare systems were Catholic. Forty-six states have Catholic hospitals, with 14% of the hospital beds under Catholic ownership - exceeding 40% of the beds in some states. These hospitals, their other healthcare components, and all their providers must follow 77 rules established by the bishops - the Ethical and Religious Directives for Catholic Health Care Services (ERDs). These rules cover a huge swath of healthcare for Catholic and non-Catholic patients, including almost every aspect of reproductive and end-of-life care. The ERDs take precedent over a physician's oath or judgment. A number of laws permit physicians and institutions to provide care in accordance with their beliefs, independent of a patients' own beliefs or choices. Physicians, healthcare providers, policymakers, and patients need to understand this dynamic in the face of the reach of Catholic healthcare. Yet, there is a surprising lack of institutional transparency. Patients often do not know if their hospital is Catholic and what care is prohibited.

This book illuminates the noble origins of the Catholic healthcare system and explores how Catholic nuns, with a deep-seated commitment to service to the most vulnerable, laid the foundation of the Catholic healthcare. It immerses readers in inspiring tales of compassion and dedication, where care was not a financial transaction, but a selfless act of mercy.

As the narrative unfolds, it examines the shift from these humble beginnings to a system now characterized by broad reach, wealth, and power with important ramifications for hospitals, doctors, patient autonomy, and patient care. The book examines these institutions' fidelity to the Catholic Church's commitment to the poor and most vulnerable and their stated missions to care for that population. The hospitals' not-for-profit status affords them tax exemptions, but do these institutions give back the same amount to their communities?

More than just a historical overview or a critique, this is an enlightening guide that underscores Catholic healthcare's contributions and uncovers its weaknesses. It makes a compelling argument for re-examining policies and practices to ensure the Catholic health system stays true to its roots of service and charity.

Intended for policymakers and funders, politicians, healthcare leaders, physicians, insurers, patient advocacy groups, ethicists, investigative journalists, legal scholars, Catholic leaders, academics, and students in health administration, the book offers a comprehensive understanding of a critical facet of American healthcare. It will open your eyes to the history, rules, reach, and healthcare implications of Catholic healthcare in America. It will ask you to consider how transparent healthcare institutions should be with their patients. It will ask you to examine what it means to be a not-for-profit healthcare institution. Finally, it will ask you to examine the boundaries between the exercise of beliefs by hospitals and physicians and the delivery of healthcare in America's pluralistic society.

Hospitals and charity Religious culture and civic life in medieval northern Italy【電子書籍】[ Sally Mayall Brasher ]

This is the first book in English to provide a comprehensive examination of the hospital movement that arose and prospered in northern Italy between the twelfth and fifteenth centuries. Throughout this flourishing urbanised area hundreds of independent semi-religious facilities appeared, offering care for the ill, the poor and pilgrims en route to holy sites in Rome and the eastern Mediterranean. Over three centuries they became mechanisms for the appropriation of civic authority and political influence in the communities they served, and created innovative experiments in healthcare and poor relief which are the precursors to modern social welfare systems. Will appeal to students and lecturers in medieval, social, religious, and urban history and includes a detailed appendix that will assist researchers in the field.

Physicians and Hospitals addresses an issue of concern and one of fundamental importance to the American health care system. While the ranks of physicians continue to swell and hospitals continue to expand their facilities, federal, state, and local governments remain determined to control health care expenditures. As a result, checks on the supply of services and facilities have been implemented that strain the physician-hospital relationship, often placing physicians and hospital administrations in conflict. The implications for American health care are the subject of disagreement and vigorous debate.

Bishops and Bodies Reproductive Care in American Catholic Hospitals【電子書籍】[ Lori Freedman ]

One out of every six patients in the United States is treated in a Catholic hospital that follows the policies of the U.S. Conference of Catholic Bishops. These policies prohibit abortion, sterilization, contraception, some treatments for miscarriage and gender confirmation, and other reproductive care, undermining hard-won patients’ rights to bodily autonomy and informed decision-making. Drawing on rich interviews with patients and providers, this book reveals both how the bishops’ directives operate and how people inside Catholic hospitals navigate the resulting restrictions on medical practice. In doing so, Bishops and Bodies fleshes out a vivid picture of how The Church’s stance on sex, reproduction, and “life” itself manifests in institutions that affect us all.

Three Years in Field Hospitals of the Army of the Potomac【電子書籍】[ Anna Holstein ]

When the first sounds of war were heard, and there dimly dawned the startling fact that traitors were imperiling the life of the nation, we all remember how thousands rushed to arms at our country’s call, eager to proffer aid in this her hour of need. City, village, and country alike gave, as their first offering, their young men, the pride and strength of the land. The first that our quiet valley knew of the preparation for war, a company was being gathered from about our very doors,ーwith Col. Hartranft (now major-generalーand nobly has he won the double stars, to which his bravery entitles him) as their chosen commander. We saw them as they stood beneath the shade of a spreading tree, with uplifted hand, vowing true allegiance to the best government the world has ever yet beheld; and as that roll now shows, many upon far-off battle-fields have sealed it with their blood. They followed where Burnside led; and all along that way, which occupied four years of these eventful times, we trace their course, marked by the battles in which they so bravely bore their part. As the soldiers went out from among us, there came the yearning wish to lessen somewhat the hardships of their lonely camp life, especially when sick in hospital or wounded. What each family first began to do for their relatives and friends, soon became general; and thus by uniting together, “Soldiers’ Aid Societies” were formed. With all loyal women of the land, I worked zealously in their behalf; worked, because there was irresistible impulse to do, to act. Anything but idleness, when our armies were preparing for the combat, and we knew not who should be the first to fall, who be called widow, or who fatherless. At length the battle of Antietam came so startlingly near, that it brought before us the horrors and sufferings of war as we had never previously felt it. From our midst six women felt called upon to offer their services, for a few weeks, to nurse the wounded. Though strongly urged to make one of the number, I declined. The idea of seeing and waiting upon wounded men, was one from which I shrank instinctively. But when my husband returned, soon after, with the sad story that men were actually dying for food, home comforts and home care; lying by the roadside, in barns, sheds, and out-houses; needing everything that we could do for them, I hesitated no longer, but with him went earnestly to work in procuring supplies of food, medicine, and clothing. Through the kindness of friends and neighbors, we were enabled to take with us a valuable supply of articles that were most urgently required. Fortunately they were hurried through without delay, came most opportunely, and were invaluable. The name of Antietam is ever associated in my mind with scenes of horror.

Hospitals, Doctors, Patients: is a review of the changing ethics of the practice of medicine as seen by a physician with 50 years of clinical experience. Punctuated with episodes from the world of patient care, it emphasis the shift away from care of the patient to the treatment of disease. The change in professional attitudes is a manifestation of the change of ethics in society at large, reflected in what has been termed the monetization of medicine. This has resulted in the transformation of what was once a ministry into a scientific profession and now into a business. The book offers some suggestions as to how it might be possible to restore caring for the patient to the disappearing art of medicine.

Are Our Women and Children Safe in Our Hospitals?【電子書籍】[ P.S.Remesh Chandran ]

When medical technology advanced, it was expected that the death of people from medical diagnosis and treatment errors will decrease. It did not. The twenty five years from 1990 to 2015 witnessed tremendous growth in medical advancement but during this period the number of deaths from medical diagnosis and treatment errors also increased by roughly forty five percent. Then what is medical advancement and where is it to be found?

Deaths from adverse effects of medical treatment are increasing in the world, not decreasing. Now it is between 2,25,000 and 1,50,000 people dying every year this way. Three-fourths of these are from harmful drugs’ administration and one-eleventh of all from unnecessary surgeries conducted. Medication errors cause fewer deaths in hospitals compared to those from harmful drugs and unnecessary surgeries. An unbelievably great number among these dying are pregnant women and also unborn children who die with them.

The hospital staff responsible for maternal deaths are primarily doctors and nurses, and secondarily paramedical staff like technicians, and lastly but over everyone else in responsibility the administrators of hospitals and other treatment facilities. Doctors are a good lot generally, interested only in saving lives. A few among them are but corrupt, greedy and incompetent for various reasons. The nurses are a totally devoted and caring lot except a few rotten apples among them, and such bad nurses are usually only a five among a hundred. The technicians are mostly learned and always overworked- only a very few among them are prone to making mistakes from ignorance, perhaps more from negligence. That is the picture at least in Kerala- the state with the highest literacy level in India. Kerala’s doctors, nurses and medical technicians are sought after and much admired in many countries.

A word about them: The doctors, nurses and medical technicians of Kerala are the best in the world. The parts concerning them in this book are not intended to disparage them in anyway

Hotels, Hospitals, and Jails A Memoir【電子書籍】[ Anthony Swofford ]

The publication of Jarhead launched a new career for Anthony Swofford, earning him accolades for its gritty and unexpected portraits of the soldiers who fought in the Gulf War. It spawned a Hollywood movie. It made Swofford famous and wealthy. It also nearly killed him.

Now with the same unremitting intensity he brought to his first memoir, Swofford describes his search for identity, meaning, and a reconciliation with his dying father in the years after he returned from serving as a sniper in the Marines. Adjusting to life after war, he watched his older brother succumb to cancer and his first marriage disintegrate, leading him to pursue a lifestyle in Manhattan that brought him to the brink of collapse. Consumed by drugs, drinking, expensive cars, and women, Swofford lost almost everything and everyone that mattered to him.

When a son is in trouble he hopes to turn to his greatest source of wisdom and support: his father. But Swofford and his father didn't exactly have that kind of relationship. The key, he realized, was to confront the man-a philandering, once hard-drinking, now terminally ill Vietnam vet he had struggled hard to understand and even harder to love. The two stubborn, strong-willed war vets embarked on a series of RV trips that quickly became a kind of reckoning in which Swofford took his father to task for a lifetime of infidelities and abuse. For many years Swofford had considered combat the decisive test of a man's greatness. With the understanding that came from these trips and the fateful encounter that took him to a like-minded woman named Christa, Swofford began to understand that becoming a father himself might be the ultimate measure of his life.

Elegantly weaving his family's past with his own present-nights of excess and sexual conquest, visits with injured war veterans, and a near-fatal car crash-Swofford casts a courageous, insistent eye on both his father and himself in order to make sense of what his military service meant, and to decide, after nearly ending it, what his life can and should become as a man, a veteran, and a father.

How to Become Mother-Friendly Policies & Procedures for Hospitals, Birth Centers, and Home Birth Services【

This is the only book to present the evidence-based policies and procedures that medical and non-medical staff can use to develop Mother-Friendly Care in their facilities. The Mother-Friendly Childbirth Initiative (MFCI), developed by the Coalition for Improving Maternity Services (CIMS), is a wellness model designed to improve birth outcomes and substantially reduce costs. It is an evidence-based mother-, baby-, and family-friendly model that focuses on prevention and wellness as alternatives to high-cost screening, diagnosis, and treatment programs. The MFCI is the first and only consensus document on U.S. maternity care and is recognized as an important instrument for change in the U.S. and abroad.

The book is based on research and evidence developed by CIMS and includes 10 protocols, each with detailed policies and procedures and supporting information and resources that help implement the change to Mother-Friendly Care. Each protocol is authored and reviewed by recognized leaders in a variety of childbirth and maternity care arenas. The book traces the development of Mother-Friendly Care and describes its core tenets along with supporting statistical information. These tenets eschew practices not supported by scientific evidence. The manual includes implementation strategies for the evidence-based nursing care training programs of such organizations as Lamaze, ICEA, and AWHONN, and supports the WHO-Unicef "Ten Steps of the Baby-Friendly Hospital Initiative" to promote successful breastfeeding.

Key Features:

Provides evidence-based policies and procedures for developing Mother-Friendly Care in maternity care facilities

Presents specific guidelines that can be used as a standard to measure Mother-Friendliness and subsequently used for marketing purposes

Designed to provide childbirth educators and doulas with guidelines for promoting Mother-Friendly Care to birthing women

Assists nurses who want to promote more rapid change on their units toward Mother-Friendly Care

Supports the WHO-UNICEF "Ten Steps of the Baby-Friendly Hospital Initiative" to promote successful breastfeeding

The Invisible Work of Nurses Hospitals, Organisation and Healthcare【電子書籍】[ Davina Allen ]

Nursing is typically understood, and understands itself, as a care-giving occupation. It is through its relationships with patients ? whether these are absent, present, good, bad or indifferent ? that modern day nursing is defined. Yet nursing work extends far beyond direct patient care activities. Across the spectrum of locales in which they are employed, nurses, in numerous ways, support and sustain the delivery and organisation of health services. In recent history, however, this wider work has generally been regarded as at best an adjunct to the core nursing function, and at worse responsible for taking nurses away from their ‘real work’ with patients. Beyond its identity as the ‘other’ to care-giving, little is known about this element of nursing practice.

Drawing on extensive observational research of the everyday work in a UK hospital, and insights from practice-based approaches and actor network theory, the aim of this book is to lay the empirical and theoretical foundations for a reappraisal of the nursing contribution to society by shining a light on this invisible aspect of nurses’ work. Nurses, it is argued, can be understood as focal actors in health systems and through myriad processes of ‘translational mobilisation’ sustain the networks through which care is organised. Not only is this work an essential driver of action, it also operates as a powerful countervailing force to the centrifugal tendencies inherent in healthcare organisations which, for all their gloss of order and rationality, are in reality very loose arrangements.

The Invisible Work of Nurses will be interest to academics and students across a number of fields, including nursing, medical sociology, organisational studies, health management, science and technology studies, and improvement science.

Healthcare systems across the globe are currently facing perhaps the greatest challenges and pressures to date as the need to improve outcomes, efficiency, productivity, quality, customer satisfaction and sustainability have significantly risen. Further, the emerging focus on value-based healthcare has increased performance expectations. To improve capability to face these challenges, doctors were “transformed” by health organizations and systems into more hybrid figures, i.e., doctor-managers and most recently clinical leaders. Yet, in many cases their engagement hasn’t worked as expected or desired, and there is still much ambiguity on what is the set of expectations attached to the new hybrid role.

Providing a systematic review of previous literature about the progressively worrying challenge in transforming doctors to clinical leaders, Lega and Pirino offer a qualitative analysis of different advanced countries facing the issue of training this hybrid role. Finding improved practices applicable elsewhere, they conclude with a case study focus on the Italian system.

Developing and Engaging Clinical Leaders in the “New Normal” of Hospitals suggests specific policy and practice recommendations on how the system should evolve with regards to clinical leadership.

Health Care in Crisis Hospitals, Nurses, and the Consequences of Policy Change【電子書籍】[ Theresa Morris ]

An inside look into how hospitals, nurses, and patients are faring under the Affordable Care Act

More and more not-for-profit hospitals are becoming financially unstable and being acquired by large hospital systems. The effects range from not having necessary life-saving equipment to losing the most experienced nurses to better jobs at other hospitals. In Health Care in Crisis, Theresa Morris takes an in-depth look at how this unintended consequence of the Affordable Care Act plays out in a non-profit hospital’s obstetrical ward.

Based on ethnographic observations of and in-depth interviews with obstetrical nurses and hospital administrators at a community, not-for-profit hospital in New England, Health Care in Crisis examines how nurses’ care of patients changed over the three-year period in which the Affordable Care Act was implemented, state Medicaid funds to hospitals were slashed, and hospitals were being acquired by a for-profit hospital system. Morris explains how the tumultuous political-economic changes have challenged obstetrical nurses, who are at the front lines of providing care for women during labor and birth.

In the context of a new environment where hospital reimbursements are tied to performance, nursing has come under much scrutiny as documentation of birthsーalready laboriously highーhas reached even greater levels. Providing patient-centered care is an organizational challenge that nurses struggle to master in this context. Some nurses become bogged down by new processes and bureaucratic procedures, while others focus on buffering patients from the effects of these changes with little success. Morris maintains that what is most important in delivering quality care to patients is the amount of interaction time spent with patients, yet finding that time is a real challenge in this new environment.

As questions and policies regarding health care are changing rapidly, Health Care in Crisis tells an important story of how these changes affect nurses’ ability to care for their patients.

Service Quality in Indian Hospitals Perspectives from an Emerging Market【電子書籍】[ Sanjay Mohapatra ]

This book offers an elaborate and empirical look at service quality of hospitals in the emerging market of India. The poor quality of service is a major issue in a large number of hospitals (particularly in government hospitals), which forces patients to opt for private hospitals that are generally much more expensive than government hospitals. This book provides a comprehensive understanding of service quality antecedents in Indian hospitals. It focuses on patient satisfaction and includes valuable insights and implications for hospital management and government. The book is theoretically grounded in SERVQUAL literature and uses appropriate and sophisticated techniques and tools to analyse data. It highlights causal model development with Structural Equation Modelling (SEM) and introduces a classification model, developed using Artificial Neural Networks (ANNs), in order to benchmark specialty cardiac care. The book also deals with Support Vector Machines (SVMs) and compares the errorrates between SVM and ANN to find the best classification technique among the two. Overall, this book is a timely and relevant work that contributes to the theory, practice and policy of service quality in hospitals.

The introductory chapter consists of four sections. In Sect. 1. 1 we reveal the c- rent situation in hospitals that is faced by the management. We address the general issue of personnel scheduling in the service industry in Sect. 1. 2. Then we motivate our research by considering physicians as the scheduling object. In particular, we show the complex nature of physician scheduling in a hospital environment. The focus of the research is presented in Sect. 1. 3. Finally, we conclude the chapter by illustrating the outline of the thesis. 1. 1 General Economic Situation in Hospitals The mounting pressure in the health care industry to reduce costs is forcing hos- tals and related facilities to take a closer look at their staf?ng policies (see [111]). A primary dif?culty in reducing personnel costs, the major component of the budget, is the variability in demand and the need to assign staff to ?xed shifts. Furthermore, government run facilities, especially those in the European Union, are seeing their budgets cut in terms of real dollars despite an aging and more acutely ill patient population (e. g. , see [96]). It has been reported that up to a third of the hospitals in Germanyplan a reductionin staff (see [91]). The schedulingprocess is furtherc- plicated by the generally recognized importance of taking individual preferences intoaccount. Moreattractiveschedulespromotejobsatisfaction,increaseproduct- ity,and reduceturnover(cf. [2]). However, withoutimprovedschedulingprocedures that better match supplyto demand, the level of care that theynow providewill soon become unsustainable. 1.
